Clinical Issue and Research Questions Developed Using PICOT

Ashley Mata West Coast University, Ontario Professor Robbie Johnson 9/5/2021

Brief Topic Description As nursing students, we get to deal with first had experience of evidence-based practices and research. We must learn how to formulate questions that can further our research so we may eventually use our research in a clinical setting. In my quest for a topic for this project I choose addiction. The NIH defines drug addiction as a chronic disease and those affected have “drug seeking and use that is compulsive, or difficult to control, despite harmful consequences” (National Institute on Drug Abuse, 2021). Addiction does not discriminate against age, race, religion, or gender. There are different types of addictions someone can potentially get addicted to alcohol, sex, money, gambling, tobacco but the most common types of addiction we see in nursing is related to drug and alcohol. Background Description When someone is addicted to alcohol, tobacco, or illicit drugs they are essentially rewiring their brain to have a chemical reaction to the substances they are putting into their body. This chemical reaction is causing a dopamine change in the brain creating a sense of euphoria. (National Institute on Drug Abuse, 2021). After someone continues to use these substances that sense of euphoria makes your think it is getting rewarded. This reward system is what causes a tolerance build up in the body. So essentially a person must continue to keep taking more and more of that substance to get that euphoria feeling (National Institute on Drug Abuse, 2021). Once these brain alterations take place a person often feels less pleasure in activities, they used such as food, sex, and social gatherings (National Institute on Drug Abuse, 2021). There can also be long term effects from repeated drug use that affect brain system circuits and chemicals. “Affected functions include learning, judgment, decision-making, stress, memory, and behavior” (National Institute on Drug Abuse, 2021).

Significance of topic to nursing Nurses have always played an important role when it comes to working with patients who are hospitalized due to substance abuse, rehabs, or other addiction centers. They are a part of every step of addiction that ranges from promotion all the way through to recovery (The role of nurses in substance abuse, 2018). There are four comprehensive approaches and they promotion, prevention, treatment, and recovery. During these approaches nurses “provide information and resources to a patient, implementing education programs in the community, working on teams in treatment and recovery programs, and encountering individuals as they recover in their communities” (The role of nurses in substance abuse, 2018). According to Duquesne University other job duties a substance abuse nurse will encounter are “monitoring patient treatments, administer medications, speak with patients regarding eight programs, educate on the dangers of drugs, alcohol, or addiction, and provide support to patients” (Substance abuse, 2020).

PICOT Research Questions A minimum of three PICOT questions were made for the topic of addiction using the PICOT formula set. These are my questions that I formulated

In drug addicted patients, what is the effect in a 12-step program compared to a patient not in a 12-step program affect sobriety within the first year of trying to become sober?

In drug addicted patients how does patients detoxing in rehab compared to a patient not detoxing in rehab influence sobriety over a five-day period. Does addiction in younger adults 18 years or younger influence relapse

In drug addicted patients younger than 30, what is the effect in learning compared to drug addicted patients older than 30 affect development withing the first 3 months of drug use?
