I would use a “high-risk fall” armband or bracelet and fall risk room sign up to alert staff for increased vigilance and mobility assistance. I am also responsible for routinely assisting my patient with toileting as well as assisting with other activities that the patient is unable to do on her own. Pretty much, my role is it to ensure safety, prevent patient from falling and ensure that other staff members are also aware to take precautions.

Discuss two evidence based practices that you incorporated into your care of Edith Jacobson and how these practices impact patient outcomes.

Educating the patient on getting up slowly, remain seated several minutes before standing up, or flexing feet upward several times while sitting will decrease the risk of dizziness caused by orthostatic hypotension. EBP: always have the client dangle at the bedside before standing to evaluate for postural hypotension. Watch the client closely for dizziness during increased activity. Postural hypotension can be detected in up to 30% of older clients. These methods can help prevent falls as well as maintain adequate fluid intake (Tinetti, 2003; Goodwin et al, 2014)



Move items used by the patient within easy reach, such as call light, urinal, water, and telephone- items that are too far from the patient may cause hazard and can contribute to falls.

Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Edith Jacobson’s case to an actual patient care situation. I would make sure to use proper precautions with a patient who is at risk for fall. I would make sure to call for assistance with repositioning the patient to prevent further injury, as well as preventing back injury for myself and anyone who assists in repositioning the patient....
