A neurological test was performed and patient had bilateral strength in the upper extremities. Pedal pulses were present in both lower extremities but neuro test wasn’t performed in left leg due to surgery. Tenderness was found in the left hip area and the left leg was noticed to be shorter that the right leg. Normal elasticity in the skin, color WNL, and no sweating. 2. Document your focused skin assessment of Edith Jacobson. a. The patient’s skin has normal elasticity. There are no signs of sweating and her color is WNL. 3. Document the patient teaching that Edith Jacobson would need related to fall risk and prevention during her hospitalization and upon discharge. a. Ms. Jacobson received patient education that included making sure that she doesn’t get out of bed without calling for assistance; when getting out of bed, she is to sit on the side of bed first to ensure that she isn’t dizzy or doesn’t her orthostatic hypotension; ensure that sure has on proper foot wear and ambulation devices such as a cane and socks with safety grips on the bottom. Upon discharge, her home would need to be assessed for safety to make sure that are no fall hazards such as small area rugs or grab bars in the bath room. 4. Document the essential elements of proper body alignment for Edith Jacobson and include the specific supplies that would be needed. a. The essential elements of proper body alignment include placing pillows or a wedge behind a patient’s back to reposition a patient or their left or right side every 2 hrs to promote circulation. SCDs can also be used to prevent blood clots. Extra pillows can be placed in between the knees to keep the hips in alignment. Remember to pull the shoulder that they are laying on out from underneath them to keep the alignment and help the patient remain comfortable.
