Description

In GTPAL, the GTPAL is described by numbers separated by hyphens. The G refers to the number of times the client has been/is pregnant. The T refers to term births (after the completion of 37 weeks gestation). The P refers to premature births (20 to the completion of 37 weeks gestation). The A refers to abortions (spontaneous or elective) before 20 weeks The L refers to currently living children. Using the GTPAL approach Read the Pregnancy History for each client, and Fill in the Blanks. This should help: Gravida - Term - Preterm - Abortions - Living Children = G-T-P-A-L 1. Client #1 23 year old, currently pregnant at 35 weeks, has had one miscarriage of twins at sixteen weeks, one abortion:

2. Client #2 32 year old, currently 23 weeks pregnant, comes to the prenatal clinic. She has three children at home (one born at 30 weeks and that survived, and the remaining were full term and living). Additionally, she had one still birth at 35 weeks, and one ectopic pregnancy that was surgically removed:

4. Client 4 A pregnant woman comes to the clinic for a visit. This is her third pregnancy. She had a miscarriage at 12 weeks and gave birth to a son, now 3 years old, at 32 weeks. Using the GTPAL system, the nurse would document this woman’s obstetric history as:

5. Client 5 A nurse is collecting data during an admission assessment of a client who is pregnant with twins. The client has a healthy 5-year old child that was delivered at 37 weeks and tells the nurse that she doesn’t have any history of abortion or fetal demise. The nurse would document the GTPAL for this client as:

6. Client 6 A pregnant client is making her first Antenatal visit. She has a two year old son born at

40 weeks, a 5 year old daughter born at 38 weeks, and 7 year old twin daughters born at 35 weeks. She had a spontaneous abortion 3 years ago at 10 weeks. Using the GTPAL format, the nurse should identify that the client is:

In an obstetrical history the gravida (G) and para (P) status of a woman is often written in abbreviated form. Where: Gravida is the number of pregnancies a woman has had. A multiple gestation counts as a single pregnancy. Para is the number of completed pregnancies beyond 20 weeks gestation (whether viable or nonviable). A multiple gestation counts as a single birth.
