HPI Assignment Amanda Nations Chief Complaint: Sinus Pressure Onset: 1 – When did the sinus pressure start? 2 – Was the onset sudden or gradual? 3 – Did the sinus pressure start after you were working outside or after exposure to any dust or smoke? (Ryan, 2010) Location: 1 – Do you feel sinus pressure on both sides of your face and nose? 2 – Is the pressure worse to your forehead or below your eyes? 3 – Was there any trauma to your nose or face before the pressure started? Duration: 1 – Does the sinus pressure come and go or is it always there? 2 – How long has this been going on? 3 – Have you ever had this problem before in the past and if so, it is the same type of pressure? Character: 1 – Does it feel like your head is throbbing? 2 – Does the light affect the pressure? 3 – Do you have a dull, aching, sharp, or throbbing headache? Alleviating/Aggravating factors: 1 – Does anything you do help relieve the pressure like blowing your nose? 2 – Is there anything you are doing that makes the pressure worse? Does the “pain above and or below the eyes is intensified by leaning forward or straining.” (Druce, 2000). 3 – Have you used any medications that help? Antihistamines, decongestants, analgesics? Radiation: 1 – With the pressure, do you experience any sinus drainage or post-nasal drip? If postnasal drip is present, is it causing any nausea? 2 – Does the pressure in your sinuses affect your vision?

3 – Do you have any problems with your ears? “There may also be complaints of popping or clicking in the ears, muffled hearing, nasal congestion, and halitosis.” (Druce, 2000). Timing: 1 – Does the pressure get worse throughout the day or is it worse first thing in the morning or at night? 2 – Are you doing anything specific when you notice the pressure? 3 – When the pressure starts, how long does it typically last? Symptoms associated: 1 – Do you have headaches with the sinus pressure? 2 – Are you coughing or experiencing a sore throat? 3 – Do you have any nasal drainage? “Nasal secretions turn from clear to cloudy and then to yellow or green.” (Druce, 2000).
