The UA personnel notifies you that the pt’s LOC is diminished compared to earlier in the shift. Prioritize the steps of assessment & intervention r/t this pt’s change of mental status. a) Check responsiveness & LOC. b) Administer O2 if needed & check pulse ox readings. c) Take VS d) Obtain a blood glucose reading. e) Check the patency of existing IV lines. f) Check electrolyte values. g) Check serum ammonia level. 3. A client is admitted to the hospital for complications r/t metastatic liver cancer. For each identified nursing problem, develop a client goal & nursing interventions w a rationale. Impaired skin integrity r/t to peripheral edema, ascites, & pruritus AEB report of itching; areas of excoriation d/t scratching; taut, shiny skin over edematous areas  Goal: o The patient will have decreased peripheral edema and pruitis on day of care.  Nursing Interventions o Have the pt elevate their legs to help decrease the edema o Take medications for edema, purities, and ascites o Have the pt participate in ROM exercise and walking to get the blood flowing o Make sure the pt isn’t itching the skin, moisturize daily o Make sure the pt isn’t having too much sodium, and make sure to take diuretics to help reduce the fluid in the body. o Check pts vitals Excess fluid volume r/t portal HTN & hyperaldosteronism AEB weight gain, dependent edema, ascites  Goal: o The patient will have a controlled BP on day of care.  Nursing interventions: o Take the patients vitals o Give medication for HTN and diuretics to help get rid of extra

fluid o Check pts electrolytes to make sure body isnt wasting too much potassium and isnt retaining too much sodium o Have pt exercise o Record pt I&O and sodium intake o Assess what pt knows about their condition 4. The nurse is reviewing the med list for a pt who has been diagnosed w pancreatic cancer. For each med identify the reason prescribed for the pt & action of drug.  Omeprazole 20 mg daily by mouth o This drug is taken to help ↓ the amount of acid your stomach makes. It can help prevent cancer of the esophagus, acid reflux, heartburn, etc.  Pancrelipase 1 capsule w each meal or snack by mouth o This helps to digest fats, carbs, and proteins. Pts need to take this life long. It helps to assist with nutrition. Slide 12. This pt is likely to have their Pancrease removed so they need this life long repleacement.  Ondansetron 8 mg Q8H PRN for nausea by mouth o This is used to help prevent nausea and vomiting and this patient will need it from the side effects of the chemo and radiation....
