Social History and Baker Act Legal status -Law Enforcement Baker Act - Held for up to 72 hours -Previous suicide attempt by swallowing all the pills found in a medication cabinet belonging to the foster parents. The client admits having suicidal ideation with a plan. The plan is to jump off the tallest building possible.

Step 1: Three Word Registration - able to complete Step 2: Was able to draw a clock within 3 minutes Step 3: Three word recall - had trouble remembering the last word

-Client is homeless, does not have a job nor goes to school.

Signs & Symptoms

1. Construct a no-suicide contract between the suicidal client and nurse 2. Patient will join family in crisis family counseling or attend a self- help group in the community 3. Patient will agree and uphold his medication schedule 4. Patient will state that he wants to live

Assessment Behavior

-SSRI Fluoxetine (Prozac) - Cause fewer bothersome side effects and are less likely to cause problems at higher therapeutic doses -Xanax- anti-anxiety

Plan for Discharge Patient is being discharged from the hospital after a LEO Baker Act. Patient is 21 yo and has a diagnosis of major depression was admitted 7 days ago after having plans to jump off a high building and is being discharged today. The treatment plan includes taking an anti depressive Prozac and an anti anxiety Xanax and he is being monitored by his psychiatrist and has weekly appointments to meet with his therapist. The patient is no longer depersonalized, appears content, with moderate insight, judgement, and satisfactory concentration. The patient has the plan of moving in with his foster parents until he gets back up on his feet.

The client is depersonalized, appears at, with poor insight, judgement and poor concentration. Admits to having a l f i id
