Elliana Ramirez 11/12/20 Kim Johnson Guided Reflection Questions Opening Questions How did the simulated experience of Kim Johnson’s case make you feel? It made me feel better I was more prepared this time due to the labs of catherterization we completed this week. I like practicing on a virtual simulation because I know what to do in a scenario where I broke sterility. I discarded the catheter and started a new one. I learned what to educate my patient on during this procedure and what questions I needed to ask my patient. Talk about what went well in the scenario. What went well in this scenario is that I was able to know to discard the contaminated catheter and apply a new one without breaking sterility. This made it so I did not cause my patient an infection if I did insert a contaminated catheter. Reflecting on Kim Johnson’s case, were there any actions you would do differently if you were to repeat the scenario? If so, how would your patient care change? The actions I would have done differently is that I would have ask the patient if she had any urgency, pain, or fever which are symptoms of UTI. I would have asked the patient if she felt any pain at the time. I would have asked if her urine volume and color been normal. This would change my patient care by knowing more information about how the patient is doing before administering the straight catheter to make sure she doesn’t have any other issues. Scenario Analysis Questions PCC What priority problem(s) did you identify for Kim Johnson? The priority problem that I identified for Ms. Johnson was bladder management due to the fact that she was paraplegic and she could not feel anything in her lower extermities. We do not want her bladder and kidneys to become impaired so bladder management would help keep those organs healthy. Educating the patient on the importance or the bladder management program and educating the importance of documenting all the patients input and output.

I would correct the nurse’s actions specific to the contamination of the sterile field by making sure that the catheter kit was discarded fully along with the sterile gloves, wash their hands, and get a new catheter kit and administrate it again. Learning where the mistake occurred to make sure it does not happen again. QI/I

Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Kim Johnson’s case to an actual patient care situation. The knowledge I would apply to an actual patient care situation is that I would make sure I come prepared into the room with an extra catheter kit and sterile gloves just in case I break my sterile field to prevent infection from occurring to the patient. I do not want to be unpreparded and look like I do not know what I am doing if I have to leave my room to get more supplies that would be unprofessional. The knowledge that if a patient is paraplegic that it is best to do a bladder management program to keep their health of the bladder and kidneys intact. Making sure to educate on bladder management and the importance of input and output measurements.
