Description

PSYCHOLOGICAL EVALUATION REPORT STRICTLY CONFIDENTIAL

I.

IDENTIFYING INFORMATION

Name: Lisa Rowe Gender: Female Date of Birth: Unknown Chronological Age: 20 years old Nationality: American Occupation: None Civil Status: Single

II.

REASON FOR REFERRAL Lisa has been in a mental institution in Claymore for eight

years. There is no progress in her mental illness since she always sneaks out and refuses to take all her medications. She is the longest patient that has stayed in Claymore, and she has a close relationship with every patient in their ward. However, Lisa's life decisions were all failed, which is why she continues to go back to the hospital.

III.

HISTORY OF PRESENT ILLNESS Lisa's

illness

occurred

after

the

incident

wherein

she

lighted a match inside their house. Few details were mentioned about the history of Lisa's condition. In a span of eight years, 1

her

mental

illness

has

not

changed

because

she

manipulates

herself and never listens to anyone in their ward.

IV.

BACKGROUND INFORMATION

Family There is no information mentioned about the client's family history.

Personal Lisa continues to sneak out into the mental institution, but the police continuously catch her, which is why she keeps on coming back to Claymore. She can build a great relationship with other people even if she feels manipulative and insensitive. Lisa mentioned that no one gives her attention since she was a kid, and they want her to be dead. She refers to the nurses as fools, and she keeps on telling her new friend named Susanna not to follow those nurses. She knows that something is wrong with her mental

condition,

but

Lisa

continues

to

refuse

to

take

her

medications and therapy seriously. In fact, she is proud that she was diagnosed as a sociopath. She is a happy go lucky woman who does not consider her illness as worse. Lisa brings fantasy and manipulative behavior while saying harsh words toward people. V.

BEHAVIORAL OBSERVATIONS Lisa wore a fur coat and pants when she got caught by the

police and returned her to Claymore. Her face looks indecent and dirty. She noticed that her best friend was not in the mental institution anymore because her best friend got depressed when 2

Lisa

sneaks out

of

the mental

institution.

Her clothes

were

mainly plain t-shirt and denim pants. Lisa never wore anything aside from those kinds of clothes. In terms of her tone of voice, she speaks fast and loud. It sounds like she is threatening everyone,

and

she

often

laughs

for

no

reason.

If

the

other

patients were sitting quietly, she would use a hand puppet and mimic

a

child's

voice

while

catching

the

other

patient's

attention. Lisa has the most powerful personality in their ward. She never follows anyone, even the nurses in the hospital. Also, she is proud that she is a sociopath, and she continues to practice and do what is running in her mind. However, she is an attractive woman who loves to smoke. She has a lack of empathy, social awareness,

and

regard

for

consequences.

She

became

Susanna's

friend, and they did a lot of fun things together inside the ward. Her

past

sexual

partner's

wife

confronted

her

friend

Susanna, and Lisa helped Susanna in that situation. She is a great friend. However, when Lisa and Susanna planned to sneak out of the institution, they visited their other friend named Daisy. Lisa confronted Daisy since she's been cutting her wrist and asking for some medications. Lisa said harsh words towards her, and Daisy hanged herself on the ceiling the next morning. Susanna was so shocked, but Lisa has no feelings, and she gets the money from Daisy's pocket. VI.

The occurrence of antisocial behavior is not exclusively during the course of schizophrenia or bipolar disorder. Lisa experienced all of the criteria mentioned above. All of her friends were afraid because she can do things, whether it is right or wrong, without being affected emotionally. She does whatever she wants and does things that most of us would find unacceptable. She violates social norms and expectations without the slightest sense of guilt or regret. Lisa was diagnosed with Antisocial Personality Disorder. 4

information mentioned about Lisa's family history, and we cannot conclude that she got the mental condition from her parents. It is

possible

that

her

continue to refuse to seek help. Lisa mentioned that she was treated poorly when she was a child, and everyone wanted her to die. This means that Lisa was abused and neglected during her childhood.

VIII. RECOMMENDATIONS 1. Individual

and

Group

Therapy.

Lisa

can

practice

her

personality by socializing with other people while attending a therapy session. It will help her to change her life perspective and change her character in general. Lisa should attend

It will help the therapist to notice which aspects Lisa needs to improve. 4. Drugs. Psychiatrists may prescribe medication for Lisa to improve

her

personality

and 5

give

some

pills

that

will

enhance her mood. In this way, it would be easier for Lisa to perform other tasks if she has a good mood and can control her anger.
