1. Choose one day to listen to a handover. On a separate page answer these questions about each of the clients that you heard about at the handover. (30 marks) Write: • • • •

Patient diagnosed with; Mets SCC back, both axilla MRSA SCC- Squamous cell carcinoma is a form of skin cancer that is the second most common type and it is generally found in men more then women in a ratio of 2;1. The abbreviation of Mets is referring to the skin cancer been spread through out the back of the patient, since the word back was added referring to back. Both axilla MRSA; is referring to both of the armpits been diagnosed with methicillin resistant staphylococcus aureus (MRSA) which is a form of bacteria that is resistant to antibiotics. This patient is a 60 year old male, which is at a higher risk to be infected with SCC as mentioned due to gender and also age plays a role with this type of cancer. Patient was placed in palliative care due to his illness that is very unlikely to be cured as both diseases have failed to be cured via any medical procedure.

The medical abbreviations used on the handover sheet for this patient were; • • • •

SCC- (Squamous cell carcinoma) MRSA-( methicillin resistant staphylococcus aureus) Cont- (continent, able to use toilet without aid) Transfer Ax1- (self move in wheel chair)

2. How is patient confidentiality maintained in regards to a verbal handover and the handover sheets? (5 marks) During hand over all nurses are gathered in an area close to the office when no patient or visitor will be close by and the discussion of patient cases will be processed. All handover sheets are to be placed in pocket or somewhere safe in order to maintain patient confidentiality and this is also the reason of why verbal handover is done in an area away from any person not staff or not required to listen to handover.

When verbally discussing patient information a low voice was constantly used Handover sheets were very highly taken care of and removed out of the reach of unauthorized people. When wanting to discuss with patient a certain issue/health issue, all other people in room are asked to leave including family members Patient consent is to be taken in any information delivery regarding patient, e.g. writing letter to a specialist. Any file or document with patient information and name are in the office locked safely

3. What are some of the policies and procedures that you have observed which help to maintain patient confidentiality? (5 marks)

When doctor enter room to discuss patient illness all other unauthorized people are asked to leave room. Handover sheets are not to be left anywhere where in reach of unauthorized people. Patient information is not to be discussed in front of patient or others; it must be discussed in office. The curtain must be closed when doctor/nurse dealing with patient Patient consent must be taken in regards to anything that concern their information been discussed....
