Mental Health Case: David Carter, Part 1 Documentation Assignments 1. Document findings associated with your screening of Mr. Carter using the AIMS scale. Patien couldn’t sit still he was moving a lot. Patient did not open mouth or show his tongue because he doesn’t feel safe. Patient is not having Abnormal involuntary movements. 2. Document Mr. Carter’s performance of activities of daily living and his intake and output for the day. Patient doesn’t find energy to do any activies. Mr. Carter feels tired even fter 12 hours of sleep. Patient wont eat anything unless it is sealed. 3. Reconcile Mr. Carter’s medications prior to hospitalization. Patient was taking olanzapine 10mg and venlafaxine XR 75mg daily. 4. Identify and document key nursing diagnoses for Mr. Carter. Mr. Carter is diagnosed with schizophrenia showing symptoms of paranoia, delusion, amd disorganized thinking. 5. Referring to your feedback log, document all nursing care provided and Mr. Carter’s response to this care. Patient wass assessed for a mental health examination. Support was given to patient and also Mr. Carter was brought back to relaty when he spoke about the voices he heard. Mr. Carter thought everyone wanted to poisin him but felt safier when told he ould be provided a package meal. 6. Document your handoff report in the SBAR format to communicate the care plan for Mr. Carter to the nurse on the next shift. S: David Carter, 28 year old, Was violent towards his mother when he was asked if he took meds. B: Patient is diagnosed with schizophrenia and was in a treatment regimen, taking olanzapine 10mg, and venlafaxine 75mg daily. It is noted that theses medications may have caused weight gain to the patient. He is currently having paranoid delusions thinking that people are listening to his thoughts and the auditory hallucinations are telling him not to eat hospital food.

A: Mental assessment was completed. Patient was often oriented to reality. Mr. Carter will only eat meals that are packed and closed. R: Please try to get the patient to eat and encourage patient to take medication as MD ordered. Also continue to support and orient him back to reality. Opening Questions How did the simulated experience of David Carter’s case make you feel? Talk about what went well in the scenario. Giving patient support and telling he was in a safe enviorment made he calm a bit more. Also making him aware that I was a nurse and here to help also help with his paranoia. The experience with Mr. Carter made me feel helpful and understand how to respond to the patine theraputicly.

Reflecting on David Carter’s case, were there any actions you would do differently? If so, what were these actions and why? I wouldn’t do any action differently.

Scenario Analysis Questions* PCC What issues identified in the health history could impact David Carter’s treatment regimen? The gain of weight causes Mr. Carter to not want to continue the treatment regimen.

What could the nurse recommend to the primary provider that might increase compliance to the medication regimen? It would be better to start off the patient with Third- genenration antipsychotics being that the second -generation of medication isn’t working so well. PCC/I Identify a support group that would be beneficial to David Carter’s family. Mr. Carters family should attended an educational support group that will teach them what schizophrenia is and how to react to certain situations. PCC/S What action should be initiated to promote nurse safety? Nurses should check if it is safe to enter the room before going in.

PCC Given the patient’s paranoia, which techniques would be effective in assisting the patient in performing activities of daily living? Always remind the patient is safe by providing support and also bringing them back to related with your answers.

Concluding Questions How would you apply the skills and knowledge gained in the David Carter case to an actual patient situation in different acute care units (emergency room, intensive care unit, obstetrics unit, etc.)? This vsims reminds me how important safety is when working with a patient in a menal health unit. I definetly think it is important to use this knowledge in other care units. Being that you never know if the patient isn’t correcty diagnosed. Also its important to have therapeutic communication with all your patients.
