MH Exam 2 Test Plan 1. Describe the components of a therapeutic relationship Self-disclosure can be helpful, but conversation should be about clients problems a. Empathy (not sympathy) i. The ability of the nurse to perceive the meaning and feelings of the client and to communicate that understanding to the client ii. Being able to put yourself in the clients shoes (not poor you = sympathy) iii. “gift of self”: client – by feeling safe enough to share feelings; nurse – by listening closely enough to understand 2. Discuss boundaries with respect to distance and use of touch 3. Describe the differences between: flat affect, labile mood, flight of ideas, transference, depression a. Flat affect: no facial expression b. Labile: unpredictable rapid mood swings, from crying to euphoria with no apparent stimuli; rapidly changing c. Flight of ideas – excessive amount of speech composed of unrelated ideas Ideas of reference – feel like everything that is going on is directed at you  Loose associations – jump from one subject to another and there is no relation between thoughts  Tangential thinking – can’t keep them on task, not able to stay on track  Thought process- how patient thinks  Thought content- what patient actually says 4. Identify self-awareness issues that can enhance or hinder the nurse-client relationship.. 5. What are the signs of possible suicide and what interventions can be implemented 6. Explain delusional thoughts and what are some appropriate responses a. Delusions - (persecutory, paranoid, grandiose, somatic) 7. Describe the phases of the nurse-patient relationship a. Orientation – begins when the nurse and client meet and ends when the client begins to identify problems to examine b. Working – divided into 2 sub-phases: i. Problem identification: client identifies the issues or concerns causing problems ii. Exploitation: the nurse guides the client to examine feelings and responses and to develop better coping skills and a more positive self image, this encourages behavior change and develops independence c. Termination – final stage in nurse-client relationship, begins when the problems are resolved, and it ends when the relationship is ended i. Client may feel the termination as an impending loss, often try to avoid termination by acting angry or as if the problem has not been resolved
