EVALUATION GOALS & OBJECTIVES
Health Problem: URINARY FREQUENCY
SUBJECTIVE DATA: “5 months nagstart yung maya maya ihi, hindi rin ako pala inom ng tubig. Minsan orange and minsan yellow yung color ng ihi ko tsaka minsan mahapdi, minsan hindi. “as verbalized by the patient.
OBJECTIVE DATA:
Impaired Urinary Elimination related to frequent urination as evidenced by dysuria and urinary frequency.
Short Term Goals: After 2-4 hours of nursing interventions, the client can verbalize understanding of the condition.
Long Term Goals: After 2-3 days of nursing interventions: 1. Client can achieve normal elimination pattern or participate in measures to correct it.
Dysuria
Urinary Bladder Infection
2. Occasional pains when urinating will be relieved.
Dehydration, Dry lips Experienced during the 2nd Trimester (started 5 months into the pregnancy)
3. Proper hydration can be observed.
INTERVENTIONS Ascertain the client’s previous pattern of elimination for comparison with current situation. Note reports of problems. Determine the client’s usual daily fluid intake (both amount and beverage choices; use of caffeine). Note the condition of skin and mucous membrane. Observe for cloudy, or bloody urine with foul odor Investigate reports of pain, noting location, duration, and intensity Encourage adequate fluid intake 2-4 liters per day if tolerated, avoiding caffeine, and limiting intake during late evenings & bedtime Encourage client to void every 2-3 hours Health teaching in regard to the importance of proper hygiene such as hand washing and perineal care, as well as being hydrated. Instruct client to wipe perineal area from front to back
