Priority 1: Fluid Volume Deficit (Blood, Electrolytes, fluid) M: 0.9 % NaCl: Bolus 999 mL/hr I: Assess IV site for patency, assess fluid volume status of patient, assess blood pressure, assess lung sounds, assess for edema, assess LOC, assess site that is bleeding and note how much fluid and blood is present, lay the patient in a supine position to assist in blood circulation, may need blood transfusion C: Get order from doctor: patient became hypovolemic, Blood bank E: Tell the patient that we are giving fluids to help Increase her blood pressure; Tell pt to call nurse if They have SOB, produce a cough, or chest pain Priority 2: Risk for Infection M: Cefazolin 1 g IVBP Q6h x 3 doses I: Make sure pt has no allergy to penicillins or Cephalosporins, monitor bowel function, assess Patient for rash during therapy, may increase AST, ALT, BUN, bilirubin, and creatinine serum C: Lab if a culture needs to be examined, Infection Prevention And Control Lead (IPC Lead), physician for proper antibiotics E: Notify nurse if pt becomes itchy, sees a rash, feels Feverish, or has loose or foul-smelling stools. This is Being given for prophylactic reasons. Educate the patient about Proper hand washing and to not touch the surgical site Priority 3: Risk for DVT M: Enoxaparin 30 mg subq every 12 hours I: TED hose and sequential compression device boots, PT as tolerated, Ambulation as tolerated, fluids, keep legs raised as tolerated C: PT, PCT, physician for blood thinners if necessary E: Purpose for TED hose, elevate your legs to decrease swelling and pain As tolerated, move your legs in bed as tolerated, tell nurse if you experience Calf pain, your calf becomes reddened, or inflamed.
