Hypoglycemia

Likely Medication/Food Interactions:

Drug-drug ACE inhibitors, antidiabetic agents (oral), ARBs, disopyramide, fibrates, fluoxetine, MAO inhibitors, octreotide, pentoxifylline, pramlintide, salicylates, sulfonamide antibiotics: May increase risk of hypoglycemia. Monitor glucose level closely and adjust insulin dosage as needed. Beta blockers, clonidine, guanethidine, reserpine: May mask signs and symptoms of hypoglycemia. Avoid concurrent use, if possible, or monitor patient closely. Beta blockers, clonidine, lithium salts, pentamidine: May cause hypoglycemia or hyperglycemia. Monitor glucose level closely. Drug-herb Ginseng: May increase drug’s effects. Discourage use together. Drug-lifestyle Alcohol use: May cause hyperglycemia or hypoglycemia. Monitor glucose level closely and discourage concurrent use.

Caution patient not to mix insulin combination products with other insulins. Alert: Warn patients not to share multidose pen with other people, even if the needle is changed, because of risk of bloodborne pathogen transmission, including HIV and hepatitis. Advise patients that hypoglycemic episodes can impair the ability to concentrate and react; advise patients to use caution while driving and operating machinery. Instruct patients to keep hard candy or glucose tablets on hand to treat mild cases of hypoglycemia. Advise patients to keep a log of glucose levels. Instruct patients on long-term sequelae of diabetes if not managed properly. Instruct patients to carry identification or wear jewelry indicating that patient has diabetes.
