Wash hands. Greet and identify the patient. Explain procedure to patient. Provide privacy. Ask patient how he/she feels and observe the environment. Observe non-verbal cues, mobility, and ROM throughout assessment. LOC

Task: CN I assessment of smell and airway patency, occlude one nostril then other nostril CN II Eyes: Distant and near vision~ Snellen Acuity (eye level 20 feet, cover one eye then other) Jaeger (1 foot from eye, read ~ can use a newspaper or menu if card not available) CN III, IV, VI: Extraocular movement (EOM) –six cardinal fields using finger, PERLA: Pupils Equal Round Reactive to Light and Accommodation, check sclera and conjunctiva CN V Facial sensation with cotton ball, muscles of mastication (palpate during jaw clenching) CN VII: Facial movement: raise eyebrows, smile, frown, pucker, puff out checks CN IX, X: Gag reflex, uvula movement (say Ah) and midline position CN XI: ROM Sternomastoid and trapezius muscle strength (shoulder and head resistance) Patient shrugs shoulders and turns head side to side CN XII: Inspection of tongue for deviation and/or tremors, oral cavity mucous membranes, lesions, teeth, dentures, odor, swallow. Thyroid palpation: located below Adam’s apple Carotid pulse palpation: just below angle of jaw Pass/Fail Fortis College: 1.2015
