course schedule for due dates. Students are required to turn in the assignment in order to attend lab. These are considered lab prework. All must be completed to pass the course. Clinical Reflective Journals (Pass/Fail) Students will complete two reflective journals during their clinical experience. Reflective journals provide a vehicle for students to critically reflect on their experiences and to facilitate professional growth and development by reexamining those assumptions and integrating new knowledge with an aim towards making a change in the situation as the student move forward. Journal entries must be typed. Submit a copy to your clinical faculty for review and feedback and a copy on Blackboard for tracking purposes. See Blackboard for further instructions and due dates of reflective journals. A care plan provides an individualized plan of care fo the client with various health conditions. Using the nursing process, it is important to use the care plan as a teaching tool that facilitates the ability to assess, develop and evidence-based plan of care and evaluate the effectiveness of the nursing interventions developed. One care plan should be based on caring for a client with dementia and the other can be any client the student was assigned. The nursing care plans must include:  Developing a data cluster  Identifying 3 nursing diagnoses (1 priority, 1 at-risk, and 1 health promotion)  Developing 1 Long-term goal (within 3 months) for each nursing diagnosis that is measurable and time specific (SMART goals)  Identifying 1 nursing interventions for each identified goal  Providing the rational for selecting each applied nursing intervention (must be cited)  Evaluating each identified goal as either Met, Not Met, or Partially Met and what this determination is based on. If the client goal(s) are unmet, a brief explanation as to why and what steps would be taken next is required. Care plans will be submitted to clinical faculty for review and feedback during weeks 3 and 5 of the clinical rotation (Weeks 10 & 1 of semester). Care plans will also be submitted in BB for tracking purposes. Care plan #1 due in BB Friday 3/25 7am. Care Plan # 2 is Due in BB by Friday 4/8 by 7am. See Blackboard for dates due to clinical faculty, as due dates are dependent on clinical rotation. NUR204 Skills Practicum (Pass/Fail) Students will participate in hands-on learning experiences each week in the clinical skills lab. Students will participate in Vital Sign validation prior to attending clinical and in a skills practicum at the end of the semester. Students must demonstrate competency in the foundational skills included in the patient scenario and receive a Pass Score in order to progress in the nursing program. A testin slot will be assigned to each student. Please see BB for schedule ...
