Description of Skill -Permits a rapid assessment of the need for resuscitation based on five signs that indicate the physiologic state of the neonate. -Testing is done over a period of 5 minutes after birth and can be repeated in 10-20 min if score is low.

CONSIDERATIONS
Nursing Interventions (pre, intra, post) -The nurse responsible for the care of the newborn immediately after birth verifies that respirations have been established, dries the infant, assesses temperature, and places identical identification bracelets on the infant and the mother. -Asess heart rate, based on auscultation with a stethoscope; respiratory rate, based on observed movement of the chest wall; muscle tone, based on degree of flexion and movement of the extremities;reflex irritability, based on response to gentle slaps on the soles of the feet; and generalized skin color, describeda s pallid, cyanotic, or pink. Assign a score of 0, 1, 2 for every category
Outcomes/Evaluation -Score of 7 to 10 after 5 min is an indication that the baby is having no difficulty adjusting to extra-uterine life. -4 to 6 is moderately abnormal -0 to 3 is concerning and intervention is necessary.
Potential Complications Low scoring can be caused by -asphyxiation -preterm delivery -maternal drug use -infections The test itself does not have risk, it is the events prior to delivery thats leads to low scores. Untreated babies with a low score can lead to further complications and even death.
