Opening Questions How did the simulated experience of Sabina Vasquez’s case make you feel? It made me realize that I need a little more practice with what to do at the bedside when it comes to assessment. With med administration and responding to her vitals I felt fairly confident. Describe the actions you felt went well in this scenario. The actions I believe that went well in the scenario is the initial interaction with the patient and her mother. I made sure to preform hand hygiene and identify the patient and who is her caregiver. Patients past history and current illness was discussed in depth and asked all relevant questions pertaining to her asthma exacerbation and possible pneumonia.

Recognizing the acuity of the situation, how would you include Sabina Vasquez and her mother in the decision-making process? Sabrina can be involved in the decision making process by having the choice of which medication she would rather take first, if she wants to take it herself or have someone else give it to her, and if/when applicable she can decided when she wants to take her medications. Sabrina’s mom can be a part of the decision making process by being involved in making sure that her daughter is comfortable. She can help her with treatments that she might not be able to do alone like the nebulizer.

Reflect on ways to improve safety and quality of care based on your experience with Sabina Vasquez’s case. Educate the family to clean their home as much as possible to avoid the buildup of possible allergens such as dust, avoid pets that are not hypoallergenic, and stress the importance of keeping Sabrina in a smoke free environment.

T&C/I What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format. I would include why she in the hospitals and her symptoms. I would include her previous relevant history, status, age, and weight. Her most recent assessment, focusing on respiratory because that is the primary concern. And finally a recommendation of possible bloodwork and labs to update her chart. QI

As you begin to think about discharge planning for Sabina Vasquez and her family, what teaching needs to be provided? Sabrina’s mom should be aware of what asthma is, how it presents, and what is the action plan when her daughter is having an exacerbation. Education about vaccines may help prevent future asthma exacerbations and antibiotic education is very important.

Pediatric Case 4 Sabina Vasquez (Complex) Guided Reflection Questions Concluding Questions Reflecting on Sabina Vasquez’s case, were there any actions you would do differently? If so, what were these actions, and why would you do them differently? The education should be in depth to make sure that she understands her daughters condition and how to help her in an emergency. I would have asked an interpreter to be present as my Spanish language is not proficient enough to translate and the mothers knowledge is very important. Another good opportunity for education was when the nurse was administering medication, there was no option to educate the mother about the med that was being given. I believe that this is a perfect time to educate and see if the patient and mother have any questions. Describe how you would apply the knowledge and skills obtained in Sabina Vasquez’s case to an actual patient care situation. This simulation made me realize that has much as it is important to act fast, it is also very important to stay cool and collected and evaluate all options and chose that will be best for your patient. Having a game plan and not jumping ahead allowed me to make sure that I preform all the relevant actions for this patient.
