Skit Charge RN (XX): Good morning, everyone. We all are going to have a good shift today! Christian, today Mr. Gonzalez in room 406 will be discharged, please give our international patient discharge instructions, we will be sending him home with the 4 medications Dr. Burke provided. BB, you're going to obtain vital signs on patients in rooms 404-411. YY, you will be preparing the post-mortem care for Mr. James Clark in room 421, and I will be preparing Mr. William Smith in room 413 for transfer to hospice status. Let's go team!!! Obtaining vital signs of 8 stable patients: RN (CC): Okay, BB, I need you to measure the vital signs every 4 hours and calculate the intake and output of the patients from rooms 404-411 and report the vital signs shortly back to me as soon as you can. Once you complete it, I will perform the Head to Toe Assessment. CNA (BB): Hey CC, I have obtained the vital signs for all 8 patients and calculated the intake and output for them as well. Mr. Connor, in room 405, has an elevated BP of 139/90; the patients in room 404, 406, 407, 408, and 410's vital signs were all consistent and within the normal ranges. Room 409, Mr. Smith's intake is 675 mL, and his output is 550 mL. Patient in room 411, Mrs. Fischman has a temperature of 101.5 and was given ice packs to cool her down, so the temperature needs to be monitored closely in the next hours. Mrs. Brown, in room 411, has type I diabetes and is asking for her prescribed insulin, her BP was slightly elevated at 124/82. Perform Post- Mortem Care: Charge RN (XX): YY, following the expiration of patient James Clark, in room 421, can you please make sure to perform post-mortem care. Assure appropriate cultural concessions are met and remove all devices from the body. LPN (YY): Hey XX, I just finished grooming Mr. Clark. I removed all the IVs and sensors from the body and positioned him on the bed with the head slightly elevated, legs straight and arms on the sides. I turned over the room, removed the soiled linens, and removed all the extra equipment. The family has received

Docusate Sodium, a stool softener, which will be 2 tablets daily with a full glass of water. Lomotil, an anti-diarrheal agent and you will take 2 tablets four times a day. Lastly, the doctor prescribed Lasix, a diuretic, to treat hypertension, you will have to take 1 pill every day, in the morning. (CC): You have all the numbers you need in case you need more supplies or something doesn't work. Be sure to call if your stoma changes color or has an increase in pain or bleeding or any other unexpected signs and symptoms. Do not forget that you have your first follow up appointment with Dr. Burke in 2 weeks, on May 14 at 11:45 am and have the information for them as well. When you are all ready, and your form of transportation has arrived to take you home, I will call for an escort to take you to the pharmacy to pick up your prescriptions and then to the car.
