SBAR: Postpartum Hemorrhage Situation: I am calling about Patrice Hodges, room 119 in the L&D wing. Her vitals are: BP 94/66, HR 96, Temp 38c, Resp 20, SpO2 96 @ 8L NC. I am concerned about postpartum hemorrhage returning in recovery. Background: The client is G1P1, postpartum after delivering a 41wk gestation male infant. Postpartum, her fundal height is 2 finger breadths below umbilicus and her uterus is boggy upon palpation. It firms up when performing fundal massage. She has a history of asthma which is being treated with an inhaler. The client’s delivery was complicated by chorioamnionitis, which she received IV antibiotics for immediately. Her labor was vacuum assisted, and she received a mid-line episiotomy. Postpartum, she lost over 500mls of blood. A balloon tamponade was performed after uterine curettage and patient is now in recovery. Assessment: The problem is her vitals are still exceptionally low after surgery. Recommendation: Please continue to monitor the vitals of Patrice Hodges very closely to ensure postpartum hemorrhage has stopped completely and that she is returning to her baseline vitals.
