1. ADDITIONAL CASH BENEFIT The planholder' s beneficiaries shall receive 100% of the Contract Price if the planholder, w ho is w ithin the insurable ages of 18 to 60 years at the tim e of purchase of the plan, dies during the paying period or w ithin five (5) years after the full paym ent of the plan and the planholder has not reached the age of 65 upon death.

ASSIGNABILITY

2. ACCIDENTAL DEATH AND DISMEMBERMENT

You m ay assign the plan to any deceased person, provided that the balance, if any, is paid before a service is rendered. How ever, any insurance coverage provided to the transferor shall autom atically term inate.

The planholder' s beneficiaries shall receive a cash benefit equivalent to the Contract Price for accidental loss of life, loss of both hands, both feet, sight of both eyes or a com bination thereof, if the planholder, w ho is w ithin the insurable ages of 18-55 years old at the time of purchase of the plan, dies due to accident during the paying period and the planholder has not reached the age of 60 upon death.

UNRENDERED SERVICE Your beneficiary may receive cash value according to a schedule if m em orial service is not performed or your heirs, successors or assigns may opt to retain the plan for the m emorial services only.

VIEWING PERIOD OF 3 DAYS Three (3) day view ing in our accredited mortuary chapels, or in your home w hen such facility is not available.

NO M EDICAL EXAMINATION REQUIRED! Please refer to the term s and conditions of the Crem ation Plan Agreement.

3. WAIVER OF INSTALLMENT The planholder, w ho is within the insurable ages of 18 to 60 years at the time of purchase of the plan, is exem pted from paying the installm ent balance in the event of an uninterrupted disability of at least six (6) m onths during the paying period and if the planholder has not reached the age of 60 upon disability.

4. UNPAID BALANCE DEEMED PAID The unpaid balance is considered paid if the planholder, w ho is w ithin the insurable ages of 18 to 60 years at the tim e of purchase of the plan, passes aw ay anytim e w ithin the paying period and if the planholder has not reached the age of 65 upon death.
