Clinical Exercise Physiology What is Clinical Exercise Physiology? • Relatively new career field (since the late 1960s) • Found within the healthcare realm, primarily in medical settings, where exercise is used to help clients manage or reduce their risk of chronic disease • Exciting and growing profession that is more and more organized and recognized at the provincial and national levels – and with the upcoming changes in the field of healthcare, it’s an exciting time to be a Clinical Exercise Physiologist!

TOPHAT: which motor component of fitness is the CEP mainly targeting with this exercise? a) muscle strength 4 b) coordination c) aerobic d) muscle endurance 2 ANS = B for person w/ parkinsons
Clinical Exercise Physiology What is a Clinical Exercise Physiologist? • A CEP is a certified health professional who utilizes scientific rationale to design, implement and supervise exercise programming for participants with chronic diseases and disabilities • A CEP also assesses the results of outcomes related to exercise services provided to those individuals The Clinical Exercise Physiologist, Maria Fragapane, with an internship student in a group with MS (site for Observation Hours)

Clinical Exercise Physiology What are the CEP’s services? CEP services focus on the improvement of physical capabilities for the purpose of: 1.
Chronic disease management
ex. reducing dz sx, improving energy, decreasing meds etc 2.
Reducing risks for early development or recurrence of chronic diseases
ex. preventing weight gain in DM2 3.
Creating lifestyle habits that promote enhancement of health
ex, decr sedentary lifestyle and incr PA level (ex. effect on pain)

with acute pain, yes dont move BUT with chronic pain, have to move, have to exercise progressively

Clinical Exercise Physiology What are the CEP’s services? CEP services focus on the improvement of physical capabilities for the purpose of:
increasing PA level and functional fitness will have a direct +ve impact on #4-6 4.
Improving the ease of daily living activities
5.
Increasing the likelihood of long-term physical, social and economic independence
6.
Facilitating the elimination of barriers to habitual lifestyle changes through goalsetting and prioritizing

Potential Barriers: equipment
not adapted at the gym, meds = side effects = not enough energy to train, fear of making their condition worse (inc sx), lack of motivation, transportation, $$$
7
Clinical Exercise Physiology What does a CEP do? The CEP’s responsibility is to: 1. Obtain a medical history on a client 2. Administer exercise-related tests
seen in KCEP 349
3. Analyse test results 4. Design an individualized exercise prescription that meets the specific needs of the client 5. Determine short and long term goals (along with each client) 6. Use his/her expertise to assist clients to improve health- and motor-related fitness components A CEP supervising a

Where do CEPs work? •
rehabilitation centers, community centers, university laboratories or hospital-based research facilities (ex: St-Justine Hospital) • CEPs may work independently, but many times work as a team with other health professionals including dietitians, physiotherapists, occupational therapists, physicians and nurses • Typical programs: • Cardiac rehab (Chip program) • Pulmonary rehab (Sacré-Coeur Hospital) • Chronic pain (Constance-Lethbridge) • CSST/SAAQ (Jewish Rehab Hospital) • Neuromuscular rehab (Cummings and LucieBruneau centers • Cancer rehab (Hope & Cope wellness center, MUHC, new Kin position) • … and more (Children with obesity at St-Justine Hospital)

Clinical Exercise Physiology How can a CEP make a direct impact on their clients’ lives? A CEP leads clients through a comprehensive program of health education and exercise as a means in managing risk factors for chronic disease. If high BP or high cholesterol is a problem area, the CEP knows the tools of the trade to assist clients to meet program goals. The CEP will encourage and support clients to stay on track with an exercise regimen and lifestyle changes, resulting in risk factor modification, ease in performing ADLs, and making exercise an enjoyable as well as a permanent lifestyle practice.
ex. is exercise training good enough for controlling high BP? --> no, need meds as well
A CEP supervising a participant doing aerobic training
CEP = long term process towards permanent health improvement

What is the Difference between Clinical Exercise Physiology and Kinesiology?
12
6
Clinical Exercise Physiology How does CEP differ from Kinesiology? CEP is a specialization of Kinesiology CEP deals with clinical populations whereas Kinesiology deals with a variety of areas such as: • Sports performance (training for athletes) • Physical fitness (training) for asymptomatic population A CEP supervising a participant doing exercise programming for participants resistance training • ____________________________ ____________________________ with chronic diseases and disabilities

Professional Organizations A CSEP Certified Exercise Physiologist® is sanctioned by CSEP to: • Conduct pre-participation screening, administers various health and fitness assessments, prescribes and supervises exercise programs, and provides counseling and healthy lifestyle education to general populations, high performance athletes, and those with chronic health conditions, functional limitations or disabilities across the lifespan. • Accept referrals from health care professionals who are trained and licensed to diagnose and treat acute and chronic medical conditions. • Use evidence-informed behavior change models to facilitate physical activity and exercise participation, and lifestyle modification.

Professional Organizations A CSEP Certified Exercise Physiologist® is sanctioned by CSEP to: (con’t) • Use the outcomes from health and fitness assessments to design and implement safe and effective physical activity and exercise prescriptions for both healthy and unhealthy populations • Monitor the influence of medications on the response to submaximal and maximal exercise during assessments and/or training sessions. • Measure and monitor heart rate, electric activity of the heart (using ECG) and blood pressure at rest, during exercise and post-exercise. These measures can be used to identify, but not diagnose, irregularities during rest, sub-max, and max exercise. • Conduct group physical activity sessions with screened participants. Sessions can include any one or combination of aerobic, resistance, balance or flexibility exercises within the CEPs individual knowledge competency. 22
