Description of Skill Administering an enema to a patient/client safely.
Indications - Constipation - Preparing for surgery - Stimulate peristalis -
Outcomes/Evaluation - Safely adminster edema - Patient does not feel dicomfort during procedure. - The desired intention of the edema is obtained.
Potential Complications - Enema is raised too high above patient. - Client expresses discomfort or urger to defecate during procedure. -Client experiences nausea, dizziness, and clammy skin during pricedure. - Enema is insurted too far. - Patient experience pain while edema is being placed. -Patient experience abdominal discomfort.

CONSIDERATIONS
Nursing Interventions (pre, intra, post) - Perform hand hygiene and collect material (edema, bedpan, etc.) -Adjust bed. Position patient in lefts sims position and place waterproof pad under buttocks. -Patients with limited limited bowel control are placed with bedpan under them with back of bed raised. - Warm solution to 99 to 106 degrress farenheit. Add sulution to bag and prime tubing an elevate bag above client no higher than 18 inches. - Apply lubricate to end of rectal tube, 2 to 3 inches. - Explain to client what you are about to do, them rais buttocks. -Slowly insert rectul tube into anus, pointing toward umbilicus and advancing 3 to 4 inche (in adults). Instill solution over 5 to 10 minutes. - Assess patient for any side effects and stop sultion iimmediately if observed. - Once solution is administered, clamp tube and repove slowly wrapping it in paper towel onse removed. - Instruct client to retain fluid ofr 5 to 10 min with small volume and 30 for oil enema. - Assist client tp sitting position on bedpan, commode, or bathroom onse they have the urge to defecate.
Client Education - Explain the purpose and procedure to client/patient - Explain what possition they should lay in and how long. - Explain that they may feel abdominal discomfort and to report any discomfort during the procedure. - Explain that they need to retain solution for 5 to 10 minutes, or 30 minutes, for oil retention, after the procedure.
Nursing Interventions - The bag should be raised above client but bellow 18 inches,. - Clamp tube and lower bag and tell patient to take a few small quick breaths. - Stop procedure emediatly and asses patients if side effects occure. -The tube should only be placed 3-4inched for adults, but know the different depth for children and infants. - Remeber to place lubricant 2-3 inches up the tube, then insert slowly. -Warm solution to body temperature to prevent abdominal discimfort.
