Description of Skill Ostomy care is emptying or changing the bag when it is full and caring for the stoma or opening in the abdomen

Indications The bag is full, leaking, or damaged. Usually done every few days.

Outcomes/Evaluation Peristomal skin remains intact. Patient returns to normal life and adjusts well

Potential Complications Hernias Infection of the stoma or perisotamal skin The skin can become red and raw under the adhesives Mental complications: It can affect the patients body image and self confidence leading to depression or anxiety

CONSIDERATIONS

Nursing Interventions (pre, intra, post) Pre: Gather supplies, identify patient, hand hygiene Intra: Remove clip from bottom of bag and empty, replace clip. Intra: Remove bag from stoma. Remove stoma covering. Clean peristomal skin and inspect stoma. Replace bag. Assess peristomal skin for signs of infection. Post: Dispose of waste in proper containers, hand hygiene, document.

Client Education How and when to change or empty the bag. How to inspect for signs of infection. How to limit patient discomfort while removing adhesives.

Nursing Interventions Allow patient to ask any questions and vocie all concerns. Ensure patient understands how to replace or empty the bag.
