Description of Skill A peripherally inserted central catheter (PICC line) is a long, thin tube inserted through a vein in the arm and passed through to the larger veins near the heart. Peripherally inserted central catheter access divices can be used either short or long term infusion of perenteral nutrition, chemotherapy, or other vesicant or irritating solution. Blood and blood products, antibiotics, medication/solutions in patients with limited peripheral access thearpy that is going.
Indications For long term therapy In the need for large volume, frequent blood draws, parenteral nutrition, chemotherapy or other vesicant and irritating solutions, blood products, antibiotics, IV medications or solutions when peripheral access is limited, central venous pressure monitoring, hemodialysis.
Outcomes/Evaluation PICC line was successfully inserted, PICC line was used for the mean duration of the treatment. Patient will verbalize importance of reporting insertion site discomfort, pain, burning, throbbing by the end of shift. Client will verbalize any feeling of malaise as needed. The patient will verbalize understanding of at home care.
Potential Complications Infection, Damage to veins or nerves, Bleeding, Irregular heartbeat, Blood clots, Dislodgment, Occlusion

CONSIDERATIONS
Nursing Interventions (pre, intra, post) Pre: Check order for PICC and timing, check blood tests for platelets, check imaging X-ray and ultrasound of veins, gather supplies, inform pt. of procedure. Intra: use aseptic techniques, insert needle through skin into the vein, make incision, insert catheter, advance along vein towards the heart, check X-ray or heart monitor for placement, cap the other end. Post: check site for redness and swelling, perform hand hygiene, ensure pt. safety, perform hand hygiene before exiting room of pt.
Client Education Educate client on keeping the PICC and dressing dry, watch for signs of damage like cracks & leaks, infection prevention through hand hygiene, inform pt. that it might require frequent flushing and dressing changes.
Nursing Interventions Monitor client for report of pain or swelling at the injection site, assess for venous back flow, observe for signs of bleeding from site.
