Description of Skill Provide client with skills, information, and techniques to help reduce anxiety.
Indications Patient has a vague feeling of dread/apprehension in response to external or internal stimuli that affect their behavioral, emotional, cognitive and have physical symptoms. Ex: Panic without reason, fear of objects, unexplainable worry.
Outcomes/Evaluation Be free from anxiety No injury Be able to discuss feelings and emotions that are based around clients anxiety Be able to reduce own anxiety Do not have any anxiety attacks
Potential Complications Anxiety worsens with interventions Patient does not adhere to medication regimen Patient refuses to go to any type of community, group or counseling sessions

CONSIDERATIONS
Nursing Interventions (pre, intra, post) Perform a suicide risk assessment on client. Use relaxation and therapeutic techniques to ease client into a more relaxed setting for conducting a interview. Make sure the client feels good. Show the client possible counseling, community resources and group therapy options available to them for help
Client Education Provide education to client of when to notify PCP of worsening effects of their anxiety or manifestations. Do not self-medicate, adhere to the medication provided and evaluate current coping and stress mechanisms that help or do not help
Nursing Interventions Maintain a clam non threatening manner while working with the client, develop a trusting relationship with warm rapport by answering questions and using active listening skills. Remain with the client at all times and when levels of anxiety are severe to reassure client of their safety and security...
