Description

PROCEDURE PREPARATION 1. Perform handwashing/ hand hygiene. 2. Assembles all materials and place on the bedside chair according to use. 3. Place the folded bottom sheet with its center fold on the center of the bed. Make sure the sheet is hem side down. 4. Unfolds the bottom sheet on bed with right side up and tucks at the head part, makes a miter corner and tucks from head to foot part of one side. 5. Covers and tucks waterproof underpad with cotton draw sheet. 6. Unfolds top sheet and tucks at the foot part of mattress hem side up. Does not tuck along sides. 7. Places the upper hem about 16 inches from the head part of the mattress. 8. Let the top sheet hang on the side. 9. Move to the other side and secure the bottom linens. 10. Tuck in the bottom sheet under the head of the mattress, pull the sheet firmly, and miters the corner of the sheet. 11. Pull the remainder of the sheet firmly so that there are no wrinkles. 12.Go on the opposite side of the bed and draw the pillowcase. Fluff the pillow fitting the corners well into the case. Folds the excess pillow case on the side. 13. Place the pillow at the head part of the bed with opening away from the entrance. 14. Hang gown, towel and face towel at the head board. UNOCCUPIED OPEN BED Perform steps 1 and 2 of closed unoccupied bed. 15. Proceeds with the top sheet as in close bed, only fold back top sheet and fanfolds towards foot part or make a single flop. 16. Fluff the pillow and place at the head part. 17. Hang gown, towel and face towel at the head board. 18. Leaves the room in order POST OPERATIVE/ SURGICAL BED Perform steps 1 and 8 of closed unoccupied bed. 19. Place and leave the pillows in the bed side chair. 20. Place waterproof underpad at the center of the bed

RATIONALE To prevent the spread of infection. Organization facilitates accurate skill performance. For a smooth foundation.

The top of the sheet needs to be well tucked under to remain securely in place. Additional protection for the bed and serves as a lifting or turning sheet for an immobile client. Draw sheet serves as a protection from skin irritation. To secure both rubber and draw sheet.

Wrinkles can cause discomfort for the client and breakdown of skin. Perform steps 1 and 8 of closed unoccupied bed. 33. Place waterproof underpad at the center of the bed about 18 inches from the head part. 34. Fold back and fanfold top sheet towards the foot part. 35. Insert the pillow under the waterproof underpad at the buttocks area. 36. Fold additional cotton draw sheet and put it at the place where the buttocks will rest. 37. Perform handwashing. OCCUPIED BED Perform steps 1 and 2 of closed unoccupied bed. 38. Dons clean gloves. 39. Introduce self and verify client’s identity using agency protocol. 40. Explain procedure to the client 41. Provide privacy by closing curtains around the bed or close the door. 42.Adjust bed appropriately and drop side rails. 43. Lower back and knee rest until bed is flat.

44. Check bed linens for patient’s personal items. Disconnect the call bell or any tubes/ drains from bed linens. 45. Loosen all linen as you move around the bed, from head of the bed on the far side to the head of the bed on the near side. Move patient to side of bed and adjust the pillow. 46. Place patient on lateral position with back towards you. 47. Roll soiled draw sheet, waterproof underpad and bottom sheet towards the patient. 48. Put clean bottom sheet and roll under the patient. 49. Place waterproof underpad and draw sheet with its center fold in the center of the bed and positioned so it will be located under the patient’s midsection. 50. Open the draw sheet and fanfold to the center of the mattress. Tuck the draw sheet securely under the mattress. 51. Raise side rail. Assist patient to roll over the folded linen in the middle of the bed toward you. Reposition pillow and bath blanket or top sheet. Moves to other side of the bed and lower side rail. 52. Remove all soiled linen separately, place on the chair or the foot of the bed if chair is not available. 53. Easeclean linen from under patient. Pull the bottom sheet taut and secure at the corners at the head and foot of the mattress. Pull the draw sheet tight and smooth. Tuck the draw sheet securely under the mattress. 54. Go on the opposite side of the bed and draw the pillowcase. Fluff the pillow fitting the corner well into the case. 55. Apply top linen, sheet and bath blanket if desired, so that it is centered. Fold the top linen over at the patient’s shoulder to make a cuff. Have patient hold onto top linen and remove the bath blanket underneath. 56. Secure top sheet under foot of the mattress and miter the corners. Loosen top linens over the patient’s feet by grasping them in the are of the feet and pulling gently toward foot of bed. 57. Raise side rail. Lower bed height and adjusts the head of bed to a comfortable position. Reattach call bell. 58. Remove soiled linen and place in hamper. Remove

gloves. 59. Leave the room in order. 60. Perform handwashing/ hand hygiene....
