Currently many Aged Care facilities are using manual medication charts (printed out) This paper medication chart leaves so much scope for error as misinformation Because it goes through many hands before reaching the patient Doctor prescribes medication (has to write on the chart), the nurse then has to fax it to the pharmacy, dispensed at pharmacy, checked by pharmacist, delivered to patient, There is no alert system in place (medication running out or needing review) can cause errors
