B. Needs-Assessment Summary Demographic Assessment Sentinel City has a population of 663,862 with a median household income of $49,091. 18.9% of the residents in Sentinel City live below the poverty level. Majority of the residents are Caucasian. Approximately 80.6% are white, 10.4% are African American, 3.7% are Asian, and the remaining are of other or multiple races. 10.5% of residents are over the age of 65, 21.7% are under the age of 18, and 7.4% are under the age of 5. Sentinel City is divided into four neighborhoods: Nightingale Square, Acer Tech Center, Casper Park District, and Industrial Heights (Sentinel U, 2020). Nightingale Square consist of 103,974 residents with the highest average income of $269,550. Only 0.7% of the residents live below poverty level. Acer Tech Center consist of 168,390 residents with an average income of $166,300. Approximately, 1.5% of the residents live below poverty level. Casper Park District consist of the largest population. With 352,643 residents, they have an average income of $80,134. Approximately, 22.7% of their residents live below poverty level. Lastly, Industrial Heights consist of 38,855 residents with the lowest average income of $24,672 Approximately, 37.5% of the residents live below poverty level (Sentinel U, 2020). Neighborhood Assessment Driving through Sentinel City, litter seem to be of a major issue. Industrial Heights was of major concern for safety hazards, but litter can be seen in almost all neighborhoods. Trash piled up alongside abandoned buildings, attracting pets and rodents can cause a safety hazard for the residents. Scattered throughout the city were many abandoned buildings in disrepair. Graffiti

walls and trash along the pavement can be founded throughout. There is an estimated 20% of stray pets in the city. According to Sentinel City Healthcare System, alcohol was reported to be the top drug used in the city last year. Second drug of choice was tobacco at 24%. 18% of the population reported to abuse prescription drugs, 12% reported to use marijuana, 10% used methamphetamines, 7% used heroin, and 5% used cocaine (Sentinel U, 2020). According to City Hall, Sentinel City had the second fastest EMS response time compared to three other cities. Sentinel City’s EMS Response Team had an average of 7.46 minutes (Sentinel U, 2020). In the last year, there were 20 reported murder crimes, 312 reported rape crimes, 559 reported robberies, and 1,231 aggravated assaults. There has also been an increase in gang related crime compared to Year 1. Whether it be homicide, aggravated assault, simple assault, or robbery there has not been any significant decrease in gang related crimes (Sentinel U, 2020). Scavenger Hunt Department of Parks and Recreation of Sentinel City offers swimming lessons, nutritional courses, summer programs for kids, sport leagues, fitness courses for adults, and after school programs. There are many services offered to the elderly through the healthcare system. These include, transportation, service animals, Meals on Wheels, community centers, Medical Care Advocates, Elder Abuse Prevention Advocates, and Government Entitlement Assistance. However, according to City Hall, Elder Abuse Prevention Advocates, Meals on Wheels, and service animals are not widely used in the city (Sentinel U, 2020). Social Service Department of Sentinel City list multiple Adult and Child Services available to the residents, such as adoption assistance, Head Start and Early Head Start, Youth

Independent Living Services, Stay at Home Program, and Child Care Providers Accepting Subsidy Benefits. There are also healthcare assistances such as charitable healthcare, Medicare, Medicaid, ACA, WIC and SNAP (Sentinel U, 2020). The Better Health Clinic in Sentinel City provides contraceptive services, pregnancy testing/counseling, basic infertility services, preconception health, STD services, breast cancer screening, cervical cancer screening, and other preventative health services. Other information provided include pregnancy and abortion rates. According to the data table, abortion rates have declined since Year 1, and birth rates increased compared to Year 1 (Sentinel U, 2020). The Soup Kitchen is operated by the Interfaith Church. It is open to the community and those in need. Their source are the Commodity Supplemental Food Program, Emergency Food Program, Food Assistance, and USDA Commodity Programs. They serve over 40,000 people each month (Sentinel U, 2020). The Affordable Housing Project offers studio apartments, one bedroom, and two bedroom apartments at affordable prices. Although they’re at affordable prices, location may not be ideal. 21% of the residents have complained about factory smoke (Sentinel U, 2020). Windshield Survey Traveling through the city, it became apparent that each neighborhood differed from the other significantly. Starting out with Nightingale Square, which was the most well kempt city, the buildings were still noted to be old. Constructed out of bricks, some buildings were noted to have graffiti on them, and some even barred off to prevent trespassing. As the bus travels throughout the city towards Industrial Heights, more litter and destruction of the buildings are noted. Overall, most buildings seem to be built close in proximity. There are not a lot of open spaces throughout the city. Transportation seems easily accessible throughout the city. It’s noted

that there are many pedestrians that travel on foot or bike. There is also a train station that is available for residents to travel on. Sentinel City is an urban community. It mostly consist of buildings and not many foliage. However, there is a park located in Nightingale Square and a school playground located in Industrial Heights. There are also multiple billboards posted throughout the city encouraging multiple safety measures such as recycling, preventing drunk driving, or quitting smoking. B1. Data Analysis There were many health concerns that were noted throughout Sentinel City. The most prevalent issue was the amount of litter that can be found throughout the city. Even in Nightingale Square, which had the highest average income out of all four neighborhoods, bags of trash were found on the alleys of buildings. However, Industrial Heights was noted to have the most litter. Conversely, this neighborhood had the highest rate of homelessness, which is another health concern for the city. The lack of sanitation throughout the neighborhood can cause an increase in risk for diseases. Furthermore, it correlates with an increase is pest and rodents which in turn can increase infections and diseases. Fortunately, the mayor has plans for sanitation improvements for the city. Due to the health concern and sanitation issue, it brings attention to the number of uninsured residents, especially in the neighborhood of Industrial Heights where 37.5% of the residents are uninsured. This may contribute to the high rate of STIs as reported by Better Health Clinic. This may be associated with a lack of sexual health education or drug abuse in the city. C1. Top Three Problems Healthy People 2020 is a federal government program that strives to bring awareness to health, disease and disabilities. Top three issues that were noted in Sentinel City were prevalence

of STIs, tobacco use, and gang related violence. The most concerning issue in Sentinel City may be the high rates of STI. The fact that 92% of the city’s population over the age of 25 was diagnosed with Hepatitis B is astounding. 80% were diagnosed with HIV. Unfortunately, there are many cases of STIs that go undiagnosed. That is why Health People 2020 aim to “promote healthy sexual behaviors, strengthen community capacity, and increase access to quality services to prevent sexually transmitted diseases and their complications” (Center for Disease Control and Prevention, 2020). Secondly, tobacco use is known to cause many other health issues such as cancer, heart diseases, stroke, and lung diseases (Center for Disease Control and Prevention, 2020). Healthy People 2020 strive to “reduce illness, disability, and death related to tobacco use and secondhand smoke exposure” (Office of Disease Prevention and Health Promotion, 2020). Tobacco use is highly preventable which is why Health People 2020 strive to reduce tobacco use and improve American’s quality of life. Lastly, Healthy People 2020 have an initiative to prevent injury and violence. Their goal is to “prevent unintentional injuries and violence, and reduce their consequences” (Office of Disease Prevention and Health Promotion, 2020). This applies to the high crime rates that occur in Sentinel City. Many citizens reported being victim of domestic violence or theft. According to reports from City Hall, there were 1,231 aggravated assaults last year, 559 robberies, 312 rapes, and 20 murders. C2. Discussion of Community Resources The Public Health Department offers STI immunization for youth and adults. They also offer HPV and Hepatitis B vaccines. Moreover, the Better Health Clinic offers numerous

community services such as sexually transmitted disease services, cervical cancer screening and other preventative health services. C3. Primary Prevention Topic Reproductive and Sexual Health is the primary prevention topic that would be relevant to the prevalence of STD rates in Sentinel City. D. Reflection The assessment strategies that were applied in the simulation has brought to my attention the diversity and differences that exhibit within a single city. The fluctuation between each neighborhood are drastically different. Although my hospital serves a small community, everyone experiences a different lifestyle. An individual that lives in a lower socioeconomic neighborhood may have different priorities compared to someone of higher socioeconomic status.
