Client safety and comfort Checks orders/Kardex Verifies client identity using two identifiers. Explains procedure Attends to hygiene needs Positions client appropriately, and provides privacy. Completes procedure within 10 minutes after starting Principles/Practices of Asepsis: Preparation Performs hand hygiene. Positions self on client’s right (if right-handed) or left (if lefthanded). Cleans /wipes dry a flat surface for the sterile field. Checks sterile package for signs of contamination. Places tray on clean flat surface. Places the end of urine collection tube within easy reach. Places waterproof drape under buttocks (female) or penis (male). Opens sterile tray at waist level. Positions sterile tray appropriately and maintains it within vision at all times. Pours solution and adds additional supplies to sterile field as per aseptic principles. Avoids reaching across open tray. Positions disposal bag to avoid contamination. Dons sterile gloves (avoids contamination). Attaches prefilled syringe to catheter inflation hub and inflates, then deflates, balloon for testing. Lubricates end of catheter and places drainage end inside collection tray. Places fenestrated drape over perineum, exposing urinary meatus. Maintains sterile field and aseptic technique throughout procedure. Discards used supplies appropriately and completes hand hygiene. Principles/Practices of Catheterization: Procedure Cleanses meatus as per aseptic technique, using dominant hand to hold sterile forceps. Does not contaminate sterile hand. Uses non-dominant hand to separate labia (female) or hold penis (male) during cleansing. Dominant (sterile) hand grasps catheter and inserts into urethra. Advances catheter to recommended depth. Inflates balloon. Pulls gently on catheter until resistance felt. Attaches the drainage end of catheter to collecting tubing/bag. Positions drainage bag correctly below level of bladder and not touching floor. Catheter removal Deflates balloon completely before removing catheter. Informs client about catheter removal and follow-up care. Documents per guidelines
