Employee’s Report of Injury Form One: Incident Report This form is used to chronicle and detail facts and supporting data for incidents that do occur. It is most demonstrative for a victim to report a crime that has been committed and it allows the local law enforcement to move the case into further investigatory stages with the goal of locating the Defendant. If said form is not filled out, the chances of holding the person accountable for committing such crime will NOT be facilitated and/or investigated. As a paralegal myself, I always encourage people to make this report either in person with a LEO or through COVID-19 modifications, you can make out something similar by using the web site of local law enforcement. And, you even get a temporary report number until the matter is assigned to the appropriate criminal investigation division.1

Form Two: Suspect Arrest Report This form is used two-fold in Florida; of which can be used to report an arrest of a suspect for a crime committed in the instant. I can tell you that as a sales manager I have had personal experience with this type of report because I also act as the workplace safety supervisor and I can tell you that I have to separate fact from fiction because some tend to stretch the truth on what really happened. That is why we have camera footage in every area of the office. The restrooms have no monitoring. I love when I confront someone and then they try to wiggle out of the incident. I have to engage myself objectively and fairly, but use the camera to actually investigate the alleged incident and take the actions necessary as our worker’s compensation provider (and, general liability insurance provider too) require the video along with any narrative and written reports. Failing to complete a report of this nature will not allow our company to facilitate any injuries that may have taken place....
