Good morning, I’m Andrew, I’m a student nurse shadowing _____ today. How are you? (smiling & offering hand for a salutation) So how do you feel about your time & activities here? (smiling, inquisitive/genuine look)

Client Communication (Verbal & Non Verbal)

(Pt smiles, shakes my hand, quiet and reserved) Good morning. (Pt fold hands together; sits quiety) It’s okay. Gives me something to do. (Pt perks up and smiles at both of us) I usually roll about, up and down the hallway in my wheelchair. (Pt smiles & holds trinket close) Yes! My grandkids visit me quite often! (Pt has a big, excited smile) They usually come visit me every Thursday!

Pt seems to be shy and possibly has an introverted personality. Pt is quiet, possibly tired or complacent.

I make observations as to the mood & feeling the pt is currently experiencing. N/Y; If Yes- which one? I encourage descriptions of the activities the pt usually performs.

Pt seems to love to talk about family. Personality now seems kind & genuine. Pt exhibits positive emotions when it comes to grandkids. Can’t wait to see them. Has/is cognizant of a schedule.
