Pathophysiology Related to Client Problem Heart failure occurs when the heart muscle is unable to pump effectively, resulting in inadequate cardiac output, myocardial hypertrophy, and pulmonary congestion. The heart is unable to maintain adequate circulation to meet tissue needs.
