I Olfactory

Test for smell - occlude one nostril and ask the patient to identify a scented object e.g. soap or coffee

II Optic

Visual acuity Ask the patient to read a newspaper or magazine to assess near vision Visual fields

III - Oculomotor IV- Trochlear VI - Abducens

Eyelid opening - Eyelid normally covers 2mm of the iris Assess extra ocular eye movements and for any nystagmus Assess pupillary response – size, shape and reaction to light Observe both direct and consensual light reflexes

V - trigeminal

Ask the patient to clench their teeth and test temporal and masseter muscle strength Test corneal reflex Test sensation to the  Forehead  Cheek  Jaws Use sharp and dull(paper clip) and cotton wool

VII Facial

Test motor function by asking the patient to:  Smile/Frown  Show teeth  Puff out cheeks  Purse lips  Raise eyebrows  Close eyes against resistance Test taste on the anterior two-thirds of the tongue  Salt, sugar or lemon Patients tongue must remain protruded for this assessment

VIII Acoustic

Test hearing by whispering from 30 – 60cm in the patients ear

IX Glossopharyngeal X Vagus

Test motor function by asking the patient to open their mouth and say ‘ah’ and watch the movement of the uvula Test the patient ability to swallow Test the gag reflex

XI Spinal accessory

Ask the patient to shrug their shoulders against resistance to test the strength of the trapezius muscle Ask the patient to turn their head against resistance to the right then the left to assess the strength of the sternocleidomastoid muscle

XII Hypoglossal

Ask the patient to poke put their tongue and note if there are any deviations or fasciculation. Ask patient to push their tongue into their cheek and you place your hand on their cheek and test the strength of the tongue....
