Description

Morphine Injection; Duramorph Drug Class: Opioid Analgesics Dosages and Routes of Administration: Routes-IV, Epidural, Intrathecal Adult, IV: Initial dose of morphine is 2-10 mg/70 kg of body weight Adult, Epidural: Initial injection dose = 5 mg in the lumbar region may provide satisfactory pain relief for up to 24 hours. 1-2 mg at intervals sufficient to assess effectiveness may be given. No more than 10 mg/24 hr should be administered. Adult, Intrathecal: Usually 1/10th of epidural dose. Dose is 0.2 to 1mg for 24 hours. Action/Therapeutic Effect: • Management of pain not responsive to non-narcotic analgesics • Duramorph provides pain relief for extended periods without motor, sensory or sympathetic function loss. Contraindications: Acute upper airway asthma, bronchial obstruction, those with a depleted blood volume, those unable to maintain a satisfactory blood pressure or with concurrent administration of phenothiazines or general anesthetics. Adverse Effects: Respiratory depression, dysphoria, convulsions, toxic psychoses, pruritus, urinary retention, constipation Nursing Implications: • Must have Naloxone (Narcan) present for intrathecal administration as respiratory depression, must be monitored for 24 hours. • Morphine sulfate may be habit forming. (Drug Abuse And Dependence) • Overdoses may cause respiratory depression, coma and death. • Patients must be observed for 24 hours after final dose with IV and Intrathecal • Use extreme caution in patients with head injury or increased intracranial pressure...
