School: Grade: 4th Teacher: Ms. Robinson REASON FOR REFERRAL: To administer the Reading assessment for Esmeralda in regarding her reading achievements, and for EDSP 4320 class for practice. EVALUATION PROCEDURES: This assessment included all the components of a comprehensive evaluation required by state regulations. Information provided by Esmeralda’s parents or primary caregiver, when she is younger than 18 years of age, provide important information to the educational process. These assessment results must be compared with informal assessments, observations and interviews from educational professionals and parents in order to determine if special education services are appropriate. Amy’s primary language, racial, and ethnic background were considered prior to selection and interpretation of evaluation procedures and measures. All assessment procedures measure a limited sample of Esmeralda’s total repertoire. The selected measures should only be interpreted within the limits of their measured validity.

BACKGROUND INFORMATION & EDUCATIONAL/DEVELOPMENTAL HISTORY: Esmeralda has no emotional/behavior concerns, and according to the CALP scores, it indicates that the student is dominant in English. She is also overall in good health, and her vision is within normal limits with glasses. She also has hearing with normal limits. However, her parents have divorced in 2015 and her dad has moved out of state. She has two absences and three tardies to date. She enjoys math, but also says she doesn’t like math because it’s so hard. She does not need any assistive technology to support her. BEHAVIORS DURING ASSESSMENT: There were no behavioral problems during the assessment, had a few problems answering questions. LANGUAGE: English and Spanish MEDICAL HISTORY: Visited Child Study Center for ADHD concerns and was diagnosed with ADHD on June 22, 2018 ATTEMPTS TO EDUCATE IN THE REGULAR CLASSROOM: Use the chart below for the standard scores, descriptor and percentile range rankings.
