Expected Pharmacological Action Requires activation in the liver and kidneys to create the active form of vitamin D2. Promotes the absorption of calcium and decreases parathyroid hormone concentration

Therapeutic Use Treatment and prevention of deficiency states, particularly bone manifestations. Improved calcium and phosphorous homeostasis.

Complications Seen primarily as manifestations of toxicity (hypercalcemia): dizziness, headache, malaise, somnolence, weakness, metallic taste, nausea, PANCREATITIS, polydipsia, vomiting, weight loss.

Medication Administration PO (Adults and Children): 10,000– 80,000 IU/day (with phosphorus 1– 2 g/day).

Contraindications/Precautions Hypersensitivity; Hypercalcemia; Vitamin D toxicity; Lactation:Lactation ; Concurrent use of magnesium-containing antacids or other vitamin D supplements.

Interactions

Nursing Interventions Assess for symptoms of vitamin deficiency prior to and periodically during therapy. Assess patient for bone pain and weakness prior to and during therapy.

Cholestyramine, colestipol, or mineral oil decrease absorption of vit D analogues.

Client Education Evaluation of Medication Effectiveness Normalization of serum calcium, phosphate, and parathyroid hormone levels. Improvement in symptoms of vitamin D– resistant rickets.
