Expected Pharmacological Action Provides passive immunity to hepatitis B
Therapeutic Use Prevention of hepatitis B virus in exposed patients, including passive immunity in neonates born to HBsAg-positive mother, prevention of hepatitis B recurrence after liver transplant in HBsAg-positive patients
Complications Injection site reactions. CNS: Headache, dizziness, fever, chillsGI: Nausea, vomiting INTEG: Soreness at inj site, urticaria, erythema, swelling SYST: Induration, anaphylaxis, angioedema
Medication Administration 0.5 mL IM within 24 hr of birth, and then at 1-2 mo, and then ≥8 wk after 2nd dose or ≥16 wk after 1st dose (but no earlier than age 24 wk); if mother's HBsAg status is unknown, administer hepatitis B vaccine within 12 hr of birth. Infants born to HBsAg-positive mothers: Administer 0.5 mL of hepatitis B immune globulin IM and 1st dose of hepatitis B vaccine within 12 hr of birth; give 2nd and 3rd doses of hepatitis B vaccine at 1-2 mo and then ≥8 wk after 2nd dose or ≥16 wk after 1st dose (but no earlier than age 24 wk), respectively.
Contraindications/Precautions Anaphylactic reaction after a previous dose or to vaccine component (including yeast). Hypersensitivity to immune globulins, coagulation disorders Precautions: Pregnancy (C), breastfeeding, children, geriatric patients, hemophilia, active infection, IgA deficiency, maltose sensitivity
Nursing Interventions A 2-dose series (separated by ≥4 mo) of the adult formulation (Recombivax HB) can be used in children 11-15 yr.
Interactions Do not use within 3 mo of hepatitis B immune globulin, MMR, varicella, or rotavirus vaccines even after discontinuing product Client Education Evaluation of Medication Effectiveness Provides passive immunity to hepatitis B.

Assess: • History of allergies, skin conditions (eczema, psoriasis, dermatitis), reactions to vaccinations • Skin reactions: rash, induration, urticaria [Inline Image] Anaphylaxis: inability to breathe, bronchospasm, hypotension, wheezing, diaphoresis, fever, flushing • Can be used with hepatitis B vaccine in cases of direct contact • Written record of immunization Evaluate: • Prevention of hepatitis B Teach patient/family: • That discomfort may occur at site • To report any rash, wheezing, inability to breathe immediately • Do not obtain MMR or varicella within 3 mo of this product
