Alterations in Health (Diagnosis)
Pathophysiology Related to Client Problem Hyperactivity of the thyroid gland with sustained increase in synthesis and release of thyroid hormones.
-Activity Intolerance -Impaired Nutritional Status -Risk for impaired electrolyte balance
49 REVIEW MODULE CHAPTER ___________
Health Promotion and Disease Prevention Reduce symptoms by: -Avoiding caffeine -Reducing Stress -Smoking Cessation
ASSESSMENT
SAFETY CONSIDERATIONS
Risk Factors
Expected Findings
-Women>Men -20-40 years old -Family history of thyroid disease or autoimmune disease
Laboratory Tests
-Goiter -Bruits upon auscultation -Exopththalmos (protrusion of eyeballs from orbits, usually bilateral)
Diagnostic Procedures
-TSH levels -Thyroid antibody tests -T3 and T4 tests.
-A radioactive iodine uptake (RAIU) test -Thyroid Scan -Thyroid Ultrasound
PATIENT-CENTERED CARE
Nursing Care -Monitor for dysrhythmias -Ensure adequate O2 -Give IV fluids to replace fluid and electrolyte losses esp from N&V -Ensuring adequate rest by providing a calm, quiet -Encouraging and assisting with exercise involviing large muscle groups.
Therapeutic Procedures -Surgical therapy (subtotal thyroidectomy) -Nutritional Therapy (high calorie diet) -Radioactive Iodine Therapy
ACTIVE LEARNING TEMPLATES
-Airway obstruction after thyroid surgery is an emergency-keep O2, suction equipment, and tracheostomy tray available in the patient's room. -If exophthalmos is present there is potential for corneal injury related to irritation and dryness. -If the patient is diaphoretic, use light bed coverings and change the linen often.
Complications Medications -Antithyroid Drugs -Iodine -Beta Adrenergic Blockers -Rituximab (Rituxan)-Graves' opthalmopathy
Client Education -Patient to sit upright as much as possible to promote fluid drainage from periorbital area. -Before surgery, show patient how to support head manually while turning in bed to minimize stress on the suture line. -Encourage regular exercise to stimulate the thyroid gland.
Interprofessional Care
-Acute thyrotoxicosis: excess amount of thyroid hormones are released into circulation, life threatening emergency -Atrial Fibrilation
Endocrinologist -TeprotumumabGraves' opthalmopathy
Endocrine Surgeon Preventive Medicine Specialist
-Osteoporosis...
