ISBARR (adapted from SBAR) is a powerful tool to have clear communication for effective client care. It is a standard way to communicate medical information. It improves accuracy and cuts down on dangerous errors. ISBARR stands for Identify, Situation, Background, Assessment, Recommendation and Read Back. Please use the following acronym (ISBARR) to communicate your findings in simulation.
