KINGSFLAME MERCHANDISING E ST. 1 99 0
TO: Anastasia Czar, Head of Health and Safety Department FROM: Joanna Coyoca, Store Manager SUBJECT: Shelving Accident Incident Report DATE: April 4, 2021
On April 1, 2021, at around 4:47 PM, at the Imus Store site of Kingsflame Merchandising, a top shelving crash on two customers and one employee. Two customers, namely Georgina Soo and Anna Salazar were shopping by Aisle 11 when the top shelving containing the canned drinks crashed. One store employee, Kathryn Santiago, saw it right before the crash happens and tried to push the customers out of the way, just a moment too late. Ms. Salazar got hit by the falling cans directly from the top shelves, while Mrs. Soo and Ms. Santiago slipped and fell because of the rolling cans. Mrs. Soo and Ms. Santiago had bruises on their arms, abdomen, and legs due to the fall. Ms. Santiago also sprained her ankle when she stepped on a can while pushing Mrs. Soo out of the way. Ms. Salazar received the force of falling cans on her head, while also having the same injuries as the other two. The three of them were rushed to the nearby clinic to inspect for more injuries. The preliminary injuries addressed in the store were confirmed in the clinic plus some more under their clothes. As for Ms. Salazar, medics confirmed she suffers from blunt force trauma to the head. Fortunately, medical experts stated its' not life-endangering. After securing the welfare of the customers, we investigated why did the shelving crash. Turns out, two of the screws at the end of the shelves loosen up due to time, while the other screws were rusted. The scene of the accident matched up with the statements of the victims and witnesses, and no evidence of foul play is present. Since it proves to be an accident, the victims did not want to sue the business instead, they just want us to pay for their medical fees. 1 4 N. Nueno A v enue . For future references, I suggest a routine check-up of the shelvings to make sure that they are still durable. This shall not be limited to shelvings but also to other equipment and machinery in the store. This would prevent another accident from happening, protect our customers and employee, and would essentially benefit everyone in the long run. After all, we are passionate about our service....
