This is a skills based assessment in which you will be assessed on your ability to demonstrate skills required in the unit. For this assessment you are required to complete three (3) parts: Part 1: Answer six (6) questions relating to the case study provided. Part 2: Answer seven (7) questions relating to the case study provided. Part 3: Answer five (5) questions relating to the case study provided.

This is a skill based assessment and will be assessing you on your ability to demonstrate the skills required in these units.

Specific task instructions The instructions and the criteria in the tasks and activities below will be used by the assessor to determine whether the tasks and activities have been satisfactorily completed. Use these instructions and criteria to ensure you demonstrate the required skills and knowledge.

Part 1 Read the case study below and answer the six (6) questions that follow:

Case Study Josiah is a 24 year-old man referred by his GP for depression. The referral states that Josiah drinks 610 units of alcohol daily and is on anti-depressant medication. He has a mental health care plan and sees a social worker once a month but has recently separated from his partner and has been having suicidal thoughts. You are a case manager in a community-based mental health service and your role is to support people to develop a recovery plan and strengthen their support networks.

1. What communication techniques would you use with Josiah? Speak in neutral tones, use open questions to generate conversation, then use more focussed questions to hopefully allow the Josiah to look more closely at root causes. If answers are brief, I would then use a few prompts, such as “can you tell me more about (that)” Active listening & taking notes, ensuring to ask questions to ensure clarity of information I would want to engage in a way that allow the client to tell their story & describe what recovery may look like for them –self define & direct.

2. What steps would you take to provide and/or adapt your services to meet Josiah’s needs? Develop a Suicide safety action plan with the client Discuss any AOD usage if presenting to an appointment under the influence & follow the appropriate WHS policy in handling the situation Discuss the options of telephone/online services for appointments so that client is not potentially driving whilst intoxicated. Encourage conversations of self disclosure about Josiah’s sobriety at each appointment & his safety.

3. What information would you seek from Josiah’s GP and social worker? From the GP I would seek what medication & the dosage prescribed & brief history of when depression & co exisiting conditions were first reported to them. Would also seek history of what GP has previously & is currently recommending so that my recovery plan is a holistic, client centred approach with all supports involved. From the Social worker I would seek similar history of client – what work has previously been done & any current recommendations

4. Describe three (3) strategies that could support Josiah’s empowerment and recovery. Coaching – as the case worker, I would be trying to help the client create the best possible environment for the client to be able to connect to themselves & discover their own solutions. I would try to motivate & guide them into supports that will help with their recovery. Recovery Star & Life Assessment – identifying all aspects of your life that may impact on their wellbeing – physical, mental & spiritual self. Self assessment questionaaire is completed & can be completed at set intervals to measure outcomes. Prevention – identifying triggers & early warning signs of negative behaviours & having plans or strategies inplace to use as needed

5. Working from a client-centred perspective and using a self-directed recovery approach, describe the skills you would use to support Josiah to express his own identity and preferences. I would first ensure that I have scheduled an appointment with Josiah that is long enough to hear his story – you do not want to be rushed & cut people off. Ask him questions probing questions that help him self-discover how he got where he is & how he would like to recover, what would that look like, so that we can agree on solutions I would not use acronyms or fancy words – just well considered plain language without judgment

6. Identify your duty of care and dignity of risk considerations in working with Josiah. My duty of care considerations will be around the disclosure of suicidal ideations. I would need to make sure that there is a suicide safety plan in place that has been developed WITH Josiah. I would need to make sure that he has easy access to phone numbers such as LIFELINE or other local crisis care agencies, stored in his wallet or phone. Dignity of risk considerations will be around Josiah’s AOD consumption. Ultimately, the client has the right to decide their own personal journey & take the educated risk in continuing excessive/dangerous behaviours.

Marica is a 50 year-old woman with a diagnosis of schizophrenia. Marica lives with her elderly mother. She does not work but spends most of her time drawing and painting in her bedroom and has always dreamed of having a showing of her art in a gallery. Marica is supported financially by her mother. She doesn’t have a social network but sometimes goes to the markets on her own to look around. 1. What communication techniques would you use with Marica?

2. What steps would you take to provide and/or adapt your services to meet Marica’s needs?

3. Outline the steps you would take to support Marica to progress towards her goals.

Case Study continued… Marica wants to apply for a disability support pension (DSP) to become more financially independent but her mother is not keen to support her to do this as she is concerned the application process will cause Marica more harm than good and she may not be approved.

4. Working from a strengths-based perspective, how would you uphold Marica’s rights to access a pension?

Case Study continued… One day when you are meeting with Marica in the park, she starts showing signs of severe distress and doesn’t respond to your attempts to find out what she is experiencing. She yells loudly enough to attract the attention of people in the park and several look as if they are calling for emergency assistance on their mobile phones. 5. Considering your own skills and knowledge, identify the limits of your knowledge and skills in working with Marica in this situation.

6. Write a report describing the incident for your organisation’s records, paying attention to respectful and non-judgemental language, and describing your actions.

7. Research an appropriate referral organisation and explain how they could meet Marica’s needs.

Case Study You are a support worker in a homeless service that values person-centred care and harm minimisation strategies. The service provides short-term (up to two weeks) accommodation and, in addition, provides direct case management services to people experiencing homelessness. Your primary job is to support people to locate stable and long-term accommodation. Casey is 17 years old and has lived on the streets for the past 12 months. She was thrown out of home when she told her parents that she was gay. Casey suffers from anxiety, regularly uses marijuana, tobacco and alcohol, and occasionally smokes ‘crystal’ (methamphetamine). She has come into your service to seek assistance.
