Maternity Case 5: Fatime Sanogo Guided Reflection Questions

Opening Questions How did the simulated experience of Fatime Sanogo’s case make you feel? I felt prepared for this more than other (non-OB) simulations due to being more familiar with OB as a patient. Describe the actions you felt went well in this scenario. I knew more of what could be wrong with the pain she was feeling and her status. I knew the interventions to act on and more of what to do.

Scenario Analysis Questions1 EBP/I What risk factors did you identify in Fatime Sanogo’s case that could have led to postpartum hemorrhage? Fatime had an induction, long 3rd stage, manual placenta removal, and oxytocin. EBP

List the actions that should be initiated if administering oxytocin is ineffective. She was also given cytotek. If the cytotek was also ineffective, she most likely would need a blood transfusion.

EBP

What were the assessment findings in Fatime Sanogo’s case that indicated her condition was worsening? Her lochia was severe and her vitals were worsening. Her blood pressure and O2 sat were dropping while her heart rate and respiratory rate were increasing. She had increasing pain and could not void.

PCC

What communication techniques could be initiated to decrease the anxiety of Fatime Sanogo and her husband and provide care that is culturally sensitive? A translastor could have been provided so they fully understood what was going on.

1 The Scenario Analysis Questions are correlated to the Quality and Safety Education for Nurses (QSEN) competencies: Patient-Centered Care (PCC), Teamwork and Collaboration (T&C), Evidence-Based Practice (EBP), Quality Improvement (QI), Safety (S), and Informatics (I). T&C

How important do you feel it is to include Fatime Sanogo’s husband as a member of the health care team? Why? It is very important as he will likely have to help her care for herself and care for the baby. It will help this process if he is involved and fully understands what is going on.

Concluding Questions Reflecting on Fatime Sanogo’s case, were there any actions you would do differently? Explain. I would provide more comfort and more understanding of the situation. In simulation you are not able to really respond much to the patient as far as comfort and understanding go. Describe how you would apply the knowledge and skills that you obtained in Fatime Sanogo’s case to an actual patient care situation. I will know more of what to do in a situation involving a hemorrhorage and also be able to inform the patient more of what is happening.
