Expected Pharmacological Action Bronchodilators

Therapeutic Use Relaxation of airway smooth muscle with subsequent bronchodilation. Relatively selective for beta-2 (pulmonary) receptors.

Complications Nervousness, dizziness, shaking (tremor), trouble sleeping, headache, nausea, dry mouth, increased coughing, or runny nose may occur.

Medication Administration *Inhalant (Children 6-11 yrs old): 0.31 mg via nebulization 3 times daily (not to exceed 0.63 mg 3 times daily) *Metered-dose inhaler

Contraindications/Precautions Contraindicated in patients with a history of hypersensitivity to levalbuterol or racemic albuterol. Reactions have included urticaria, angioedema, rash, bronchospasm, anaphylaxis, and oropharyngeal edema.

Interactions Tricyclic antidepressants or MAOIs may increase risk of adverse cardiovascular reactions. Beta blockers. Increase risk of hypokalemia from potassium-losing diuretics. May decrease serum digoxin levels. May increase risk of arrythmias with hydrocarbon inhalation anesthetics or cocaine.

Nursing Interventions Assess lung sounds, pulse, and BP before administration and during peak of medication. Note amount, color and character of sputum produced. Closely monitor patients on higher does for adverse effects. Observe for paradoxical bronchospasm.

Client Education Evaluation of Medication Effectiveness Prevention or relief of bronchospasm.

Instruct patient in the proper use of meter-dose inhaler and nebulizer. Notify HCP of all Rx or OTC medications, vitamins, or herbal products being taken and to consult HCP before taking any OTC medications or alcoholic beverages. Call HCP if SOB is not relieved by med.
