PURPOSE OF MEDICATION

Expected Pharmacological Action The expected pharmacological action of insulin is to lower blood glucose.

Therapeutic Use Insulin must be used therapeutically in Type 1 Diabetes, but can also be prescribed for Type 2 Diabetes.

Complications A common complication of insulin use is hypoglycemia. Irritation and lipohypertrophy at insulin injection sites is also common. Insulin can also lower Potassium levels in the body.

Contraindications/Precautions Insulin is contraindicated in patients who have hypokalemia. Caution is also advised in patients who have decreased liver and kidney function. Patients that have hypoglycemia should also be extremely careful with insulin. Insulin hypersensitivity is a precaution.

Interactions Some medications that can interact with insulin are: beta blockers, corticosteroids, estrogens, MAOIs, Levaquin, sulfonylureas, and thyroid hormones. Additionally, alcohol can interact with insulin.

Medication Administration Insulin is generally administered subcutaneously. It can be administered via syringe, insulin pen, or subcutaneous pump. Insulin syringe and pen injections should be rotated between three sites of administration on the body.

Nursing Interventions Nurses should monitor for signs of hypoglycemic attacks when caring for a patient given insulin. Monitor blood glucose levels and potassium levels. Report any abnormal findings to doctor, such as signs of diabetic ketoacidosis. Nurse should also report any weight changes to doctor, as this could influence the dose of insulin given.

Client Education Evaluation of Medication Effectiveness Monitor patient's blood glucose levels for stabilization. Monitor for adverse effects, such as hypoglycemia, in which the dosage might need to be altered.

The client should be educated on the proper way to self administer insulin. The nurse should use the teach back method to ensure that the patient understands the correct way to administer insulin. The client should also be educated on the signs and symptoms of hypoglycemia and diabetic ketoacidosis, and what to do if these symptoms occur. Education on properly monitoring blood glucose levels should also be given. Pamphlets and printed information can be extremely helpful in this education.
