Description

Misoprostol (Cytotec): MOA: Suppresses secretion of gastric acid promotes secretion of bicarbonate and mucus maintains submucosal blood flow. Indications: Prevention of gastric mucosal injury from NSAIDs, including aspirin, in high risk patients; w/ mifepristone for termination of pregnancy (can be used to treat duodenal ulcers). Prevents GI bleed. Contraindications: Do not use in pregnancy b/c it may cause partial or complete removal of fetus induced abortion. Adverse Effects: Diarrhea, abdominal pain, miscarriage Toxicity & Management: Toxic amount in humans has not been determined; Clinical signs that may indicate an overdose are sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, fever, palpitations, hypotension, or bradycardia. Symptoms should be treated with supportive therapy. Interactions: Antacids that contain magnesium worsen diarrhea. Nursing Process: Advise patients to administer w/ meals and at bedtime to reduce severity of diarrhea; Assess patient for epigastric or abdominal pain, blood in the stool, emesis; Monitor uterine cramping and bleeding; Assess dilation of the cervix.

Gout: Gout is caused by the overproduction of uric acid or decreased uric acid excretion, or both. This overproduction and/or decreased excretion can often result in hyperuricemia (too much uric acid in the blood). When the body contains too much uric acid, deposits of uric acid crystals collect in tissues and joints. This causes an inflammatory response and extreme pain because these crystals are like small needles that jab and stick into sensitive tissues and joints. Occurs mainly in the big toe....
