ASSESSMENT

SUBJECTIVE DATA: Client verbalize “Parang may dalawang bukol na malambot akong nakakapa tas makati at masakit kapag umuupo at dumudumi naramdaman ko to nung mga 7 months Nakong buntis “. She scales her pain in 7/10.

NURSING DIAGNOS IS Acute pain related to difficulty in defecation as manifested by small distended veins, pain scale of 7/10 and facial mask of pain.

PLANNING AND IMPLEMENTATION GOALS & OBJECTIVES

INTERVENTION

RATIONALE

After one (1) hour of effective nursing intervention the patient will be able to report decrease of pain as manifested by patient will be able to:

 Identify measures to decrease pain or discomfort during defecation.

OBJECTIVE DATA: Restlessness Facial mask of pain Guarding behavior T: 36.5 PR: 88 BP: 120/90 BMI: 24.6 PS: 7/10

INDEPENDENT:  Encourage and instruct the patient to do these following measures;  Increase fluid intake  Eat food rich in fiber such as fruits and vegetables.  Eating small and frequent  Avoid eating spicy food

 Increased fluid intake and eat food rich in fiber maintain soft stools to avoid straining during defecation;  Eating small can stabilize blood sugar levels and provide nutrient throughout the day; eating frequent promote more efficient metabolism  Eating spicy can add up to discomfort because it has capsaicin that can irritate the lining of stomach and intestine.  To promote comfort and proper hygiene

Identify and prepare necessary materials and available equipment’s. - 1-2 liters of clean tap water - clean utility pale - heating appliance such as electric kittle or cooking vessel. - clean towels - soap 2. Put 1 to 2 liters of clean tap water on heating vessel or heating appliance and wait until boil 3. Put the boiled water on utility pale and add some tap water, make sure the water temperature does not cause skin burns by checking it using back of your hands. 4.
