One of the truly essential nursing responsibilities is to educate patients and their families on how to respond to their health care complications and how to encourage health. According to Miller & Stoeckel (2019, p. 4), "the overall purpose and goal of health education is to promote, retain, and restore health, which involves the prevention, treatment, and management of illness and the preservation of clients' mental and physical well-being." This paper's objective is to guide the reader throughout the patient education process from the nursing perspective. Situation Patricia Patterson, an 89-year-old patient, was recently diagnosed with Pneumonia and will require a Home Health nurse to provide daily IV antibiotics via her CVAD. She has a PICC in her right upper arm. She will need extensive teaching. Background She is 5" 4 and weighs 92lbs. She has been a Smoker and has a 30-packs year history and currently smokes 15 cigarettes per day. She has been diagnosed with COPD, Atrial fibrillation, Congestive heart failure (CHF), Hypertension (HTN), and Osteoporosis, Coronary artery disease (CAD), and had a Myocardial infarction five years ago and had a cardiac stent placed in the RCA. Her Ejection Fraction (EF) is 30%. She has fallen twice in the last two years but has not broken anything. She lives at home, in a two-bedroom apartment, on the second floor. Mrs. Patterson lives with her daughter, who is in her 50's and her granddaughter, who is in her 30's. Needs Assessment Ms. Patterson is an older adult with completed 9th grade and a 5th-grade reading level; therefore, her ability to clearly understand information about treatment, how the information applies to her personally, to make choices and express herself has to be assessed (Miller & Stoeckel, 2019). She is very nervous about her diagnosis and wants to know why this happened

and how she can prevent it. She is also worried about the "tubes" sticking out of her arm. She wants to get better and resume her normal lifestyle as quickly as possible. Before being discharged, she must be evaluated by the cardiologist, pulmonologist, ophthalmologist, nutritionist, and psychologist. The patient would have to be assessed by a physical therapist to recognize the strength and tolerance of the physical activity. A social worker has been consulted to obtain referrals for smoking cessation programs and as well as any other information. The patient will also be sent home with home health that would administer IV antibiotics through the PICC placed in her right upper arm; the home nurse must reinforce all education provided during hospital admission. Learning Style She states she has learned better in the past by watching videos, discussions, and doing hands-on activities. The appropriateness for her learning style leans toward audiovisual resources along with easy-to-understand brochures and written instructions. Readiness of the Learner Readiness of the Learner or willingness to learn "is an aroused interest or expression of curiosity that occurs when the client makes conscious choice to learn" (Miller & Stoeckel, 2019, p. 98). Based on Ms. Patterson's manifest desire to understand why this happened, how she can prevent it, her capacity to follow some health care guidelines, and due to her physiological and developmental abilities (advanced age and limited academic education), her willingness to learn can be classified as level 2 Goal Development The main goal of this educational plan for Ms. Patterson is to help her understand all the health dangers of prolonged cigarette smoking and tobacco products and the importance of

She will be provided with information and educational materials appropriate to her learning style to help her make the best decisions for her physical and mental well-being. Lastly, the nurse will ask Ms. Patterson to verbalize her understanding of the comprehensive treatment before she is being discharged from the hospital. Learning Outcomes The adherence and strict compliance with the smoking cessation program, as well as a balanced diet and aerobic exercises. Whereas, through psychotherapy, Ms. Patterson will have the possibility to express her feelings about the health process she is going through; all these tools will help Ms. Patterson to improve her state of health in a comprehensive way. Postural drainage: This technique uses gravity to help drain mucus by placing the body in specific positions to drain the five lobes of the lungs. (Mackey, Hunter, Dokhnan, Lowe, & Thomas, 2016) Instructional Methods 1. Stop smoking 2. Use nicotine replacement therapy recommended by the doctor. 3. Eat a balanced diet. 4. Exercise at least 30 min daily 5. Take medications prescribed

6. Follow up with PCP and specialists within a month. Time and Resources The home nurse will be in charge of administering antibiotics to fight pneumonia and changing the dressing once a week. Ms. Patterson will actively participate in a smoking cessation program, receive physical therapy at home twice a week, and the rest of the days, she will take short walks around the neighborhood with her daughter or granddaughter. For the next three months, she must have monthly follow-up appointments with the cardiologist and pulmonologist to monitor her heart and lung problems. Evaluation The pulmonologist performed the patient after one month; the Levaquin was discontinued and indicated to remove the PICC line. The patient has been compliant with the smoking cessation program and continues to attend psychotherapy and receive physical therapy at home.
