Cough Summary: Causes:  URTI OSCE – OTC (Respiratory) Station  Cold/dry atmosphere  Smoking  Conditions: Asthma/COPD/HF/Allergies  Medications - Acei ASMETHOD: Dry Cough (Non-productive A – Age/Appearance cough):  Dry, tickly irritation of throat S – Self or not? (If not, ask age) and chest  No sputum M – Medication Chesty Cough (Productive cough)  Excess sputum E – Extra Medicines (tried)  Thin and clear – removed T – Time Persisting by cough  Thick and difficult to clear H – History chest  Coloured sputum O – Other Symptoms

Common Cold Summary: URTI characterised by nasal stuffiness and discharge, sneezing, sore throat and cough  Symptoms within 2-3 days - Runny/blocked nose - Sneezing/coughing - Headaches/aches and pains - Temperature - Sore throat - Earache (if uncomfortable = normal, if painful = referral)

D – Danger Symptoms Respiratory Danger Symptoms:          

Chest pain SOB Wheezing Swollen Ankles Blood in Sputum Palpitations Persistent Cough Whooping cough Coloured sputum Croup (Hoarseness in voice)

Allergic Rhinitis Two types:  Seasonal – same time each year - Response to grass/tree pollen = hayfever  Perennial – occurs throughout year - House dust mite/ animal fur/dust/spores Symptoms:  Sneezing  Rhinorrhoea (nasal cavity filled w/ mucus fluid)  Nasal congestion  Nasal itch  Irritation of mouth and palate  Itching eyes with lacrimation, oedema and conjunctivitis

Influenza Summary: Symptoms of common cold PLUS:  Rapid onset  Weakness and fatigue – bed bound  Fever Complications:  Bronchitis/Pneumonia  Exacerbations of asthma and COPD  Otitis Media  Sinusitis

When to Refer Cough:       

Lasting longer than 3 weeks Blood in sputum Chest pain (PE) SOB/Wheeze Recurrent nocturnal cough Suspected whooping cough or croup Smokers with recurrent cough

Hayfever/Pollen counselling:  Avoid going out when pollen count is high  Keep windows, doors closed  Wear sunglasses  Shower and change clothes after being outside  Avoid freshly mowed lawn...
